Injured employee was attempting to install a roof bolt when the bolt bent, separated from the drill chuck, and struck EE's right forearm resulting in a fracture and laceration.

EE had laid EE's roof bolt against drill mast and realized the drill bit was stuck in hole, EE went to retrieve bill using drill steel and as EE was lowering the drill mast it came in contact with roof bolt causing it to strike employee in the mouth.
Employee just installed glue and roof bolt into hole and was helping operator put bolt into drill pot, the operator started to push bolt up into hole unaware that other employee had hand on drill pot, as EE raise boom, employee got pinched between mast and steel clamp
Employee reached up to spin 630 mucker to align to fit through work deck. They placed a hand on top of workdeck post, when the crosshead set down on the work deck smashed the finger.
