At time of injury employee was operating an electric winch. Employee was operating pendant controller guiding the winch cable onto the cable drum with right hand as the grinding cylinder was being hoisted into position, when right glove got caught on the cable and it pulled right index finger into the cable and drum. Employee has not missed anytime from work.

Showing all 12Employee was conducting Firewatch next to a conveyor head chute where one of the mechanics was welding. The employee wore PPE and was not staring directly at the welding arc. Employee went home with no issues and woke up the next day with burning eyes and irritation. Employee went to urgent care and received prescription eye drops/antibiotics and was returned to full duty.
Technician injured lifting 988 wheel loader rear differential using service truck crane with 5ton lift capacity. 2 F/S techs were picking up 8300lbs differential. The differential started to swing and f/s tech tried to slow swing using hand. F/s tech's elbow hit inner rear bumper inside wheel well. Strain/Sprain to (R) Elbow and (R) Wrist.
EE's hand was pinched and lacerated during a task in which a barrel tipper was being used to straighten a barrel. The barrel slipped and pinched EE's hand.
On 12/05, employee was jackhammering material in the baghouse; debris got under their safety glasses and in their eye. Employee reported on 12/06, and upon visiting the eye doctor, a piece of metal was removed and antibiotics were given. Employee returned to work the same day.
Miner was working above a welding operation. Miner believes they received welding burns to eyes from reflection off of steel. Reported next day that eyes were light sensitive, they went to clinic and received antibiotic drops. Miner informed clinic that this was not a workplace injury, but after investigating further it is determined that this could be a work related injury.
Miner was using an air lance when material came back at EE. Miner got cement dust in EE's left eye. First aid was provided by flushing EE's eye. Miner went home, EE arrived back at work the next day with redness to EE's left eye, EE went to local clinic for eval. EE had EE's eye flushed and received an antibiotic ointment. Miner has remained on full duties with no restrictions.
Employee was air lancing along the south wall of stage 1 to remove build-up of material. As the employee pushed forward with the air lance, they felt a pop in their left shoulder.
Employee did not realize that a piece of hot slag material had fallen into EE's boot until EE had already sustained a burn. Upon further medical evaluation on 1/11/21, the employee received antibiotic and a restriction of "sit down work only". Employee returned to work the same day. Employee returned for a follow-up evaluation on 1/14/21 and received a full duty release.
Miner was connecting a 2" air hose from the finish mill air system to a portable compressor. Miner was in the process of connecting the cable whip check when a second employee turned on the air valve at the receiver tank blasting the miner in the face with pressurized air.
Employee responding to a call to clear material blockage from the raw mill inlet chute. Opened a bolted access panel and attempted to clear material with a jack hammer bit. EE failed to lockout the equipment and the bit got caught in a moving rotary air lock and EE cut/lacerated left index finger on the access door flange.
Employee was working at the MSC to clear material build up when EE removed the protective lens on the hot suit to peer into a drilled port in the vessel. An air cannon discharged, and hot gas and material struck the exposed area of EE's face. Corneal abrasion to right eye and minor burns around eyes and bridge of nose. EE had turned off air cannon but failed to discharge it.
