After pinning the pivot bores, on a Komatsu 930e haul truck, the cranes were lowering the bed over the hoist cylinders. While lowering the bed the ear of the cylinder contacted the ear of the bed breaking the lifting eye used to retain the cylinder in position. The cylinder then contacted the left arm of technician and crushed it against the rear tire.

Showing all 11EE was torquing lug nuts on the left rear drives with a Rad nut runner on a Komatsu 980 Haul truck. The nut runner slipped out of EE's hands and landed on EE's left hand causing a contusion.
While working on the left rear outside tire and wheel assembly, EE was loosening the lugs on the wheel and holding on to the 1" impact wrench when the turning force pinched left hand between the impact wrench and the final drive (Hub) which resulted in a stable fracture of the right 3rd fingertip.
Haul truck was contacted on dovetail by the shovel bucket. Employee complained of a whiplash event and was evaluated and returned to work. Employee was seen later and diagnosed with neck strain/sprain and mild concussion without loss of consciousness and removed from work. Doctors notes were received on 8/9/23. Referred to neurologist and p/t.
Welder was sectioning out a piece of steel from truck bed. On final cut, the metal sprung up on the opposite corner striking welders hand. This resulted in 2 broken bones in the hand.
Rubber tire Operator was pushing material off Dump 1 in the west pit. The Operators hand came off the joystick controller and struck the console resulting in a fracture to the right hand 5th metacarpal bone.
Haul truck operator getting loaded by the shovel, shovel had an electrical fault. The shovel bucket came in contact with the canopy of the truck causing the truck operator's face to contact the driver's side door glass resulting in a bloody nose, neck/back strain. Was diagnosed with a Cervical strain. The first scheduled shift missed was 11/26/2021.
Mechanic had cut a piece of all-thread. While cleaning off the excess thread with the wire wheel, the wire wheel caught the piece and smashed EE's left index finger between the piece of all-thread and guard resulting in a laceration to the tip of the finger. Sutures were required to close the wound. Mechanic received a work as tolerated release and returned to work.
HT loaded by shovel, 1st bucket the door opened unexpectedly, material fell on the dovetail causing the bed to raise up, slam back down. HT driver experienced leg pain upon impact, was seen at the ER for general body aches, given a work as tolerated release. On 11/30/20 employee informed the mine operator that EE had been receiving physical therapy which started on 10/09/20.
Employee was removing lift cylinder for drill 10108 mast. Mast was secured hydraulic lines bled off, but when cylinder was cut the mast dropped 4-6 inches striking employee on hard hat. Employee went to coalition and saw Dr where EE was diagnosed with concussion and not allowed to return to work. Employee will return to work when concussion symptoms are gone.
Operator tramming track dozer 26641, encountered a rock caused operator to inadvertently set park brake. Machine suddenly stopped, operators left shoulder jammed causing a shoulder strain. Employee was given a work as tolerated release 1/2/2020, then on 1/9/2020 notified the mine that the MRI showed rotator cuff tear, taken off work 1/9/2020. Begin physical therapy 1/13/2020.
