Employee was installing center bolts using the Fletcher Roof Bolter. Had drilled EE's starter hole and just changed drill steels and when EE started drilling again, a piece of rock fell from the top and bounced off the drill head and struck EE's left forearm. Suffered a contusion with swelling. Missed 3 days of work. Has training and experience and was wearing required PPE.

Injured employee hooked a scoop winch hook to a shield on a 30-Ton car to center up the shield on the car. When the scoop operator tightened up the winch rope, the hook slipped off the shield and struck employee's right leg above the knee. They suffered a contusion/bruising, pain & swelling. Employee failed to position themselves in a safe location.

Employee was using a grinder to cut a bolt when the grinder disk came apart and struck EE in the left hand resulting in a laceration. The injury resulted in 5 sutures.

Employee was assisting installing J-screws into rib. When the bit on the drill became lodged and stuck, employee attempted to free it by pulling on the drill. Employee's hand slipped onto the bit, which spun and injured the employee's hand. Injury was not the result of a training or compliance issue.

Injured employee was roof bolting on the left side of the continuous miner. Employee was in the process of installing a rib bolt when a piece of coal/rock fell from the brow behind them and struck them on the left shoulder. Was diagnosed with a deep tissue contusion and given time off. No rules or regulations were violated.

Employee was engaged in the mucking phase of shaft construction. A bucket of muck was sent out of the shaft when a rock fell striking them in the hardhat. This resulted in the employee having a small laceration which needed 2 staples and a mild concussion.

Injured employee and co-worker were using a small crane mounted on a mantrip to lift and load a shuttle car reel on the end of the mantrip. As the reel was suspended it became stuck on the edge of the mantrip platform. The employees were attempting to free it up when the reel slipped off the hoist hook and fell on injured employee's right foot. Suffered a contusion and bone bruise.

Employee was bolting off the right side of the miner when EE was pushing a bolt up and the plate shifted. EE then reached up to adjust the plate but was still pushing the bolt up causing EE to pin EE's hand between the plate and the strap. EE had skin abrasions on EE's hand.

Employee was using an angle grinder to cut a piece of metal angle. The grinder slipped while cutting and contacted their left upper leg causing a cut. Employee received 4 stitches to close the wound. Returned to work their next shift. Employee failed to maintain control of the grinder. No rules or regulations were violated and employee has training and experience.

Injured employee was using a hand-held grinder to cut a 4" long x 1/4" diameter steel pin. As employee was cutting on the pin, a piece the griding wheel broke and the grinder kicked/jerked suddenly. The wheel contacted employee's left wrist causing a cut requiring surgery and stitches. Employee failed to properly control the tool during use.

Injured employee was hanging trailing cables on the boom of the loading machine to be advanced up the entry. They extended their right hand to hook the pull rope on the boom at the same time the boom was re-positioned. Employees hand was caught between the boom and coal rib. Employee suffered a broken little finger on their right hand.

Injured employee was roof bolting on the right side of the continuous miner. EE was drilling a hole for bolt installation and was in the process of adding an additional section of drill steel when the existing steel in the roof fell out, striking the top of EE's left hand. EE suffered a broken ring finger and a cut requiring stitches.

When the work platform was landed on bottom it placed slight tension on the entire makeup of the rebar boot. EE unscrewed the clevis and did not hold the cable it was attached to as instructed by EE's supervisor. As a result, the clevis once unattached sprung and swung striking EE on the cheek. There was no bleeding, swelling or visible injury seen.

Employee was walking alongside the longwall shear observing the cut-out process. EE stepped on a piece of coal, stumbled and began to fall. EE reached out with right hand and grabbed the handrail to catch self and at that moment the shear cable pulled on top of EE's hand. Employee received a laceration to the top of the hand and 2 broken metacarpal bones.

Employee was utilizing a straw blower to spread mulch on exposed ground. Employee stood on top of the mulcher guard which caused the guard to collapse. Employee fell onto the chute and employee's feet were struck by rotating chains. Another employee was nearby to assist the fallen employee off of the mulcher. The employee sustained fractures on both feet.

Employee was installing roof bolts on the left side of the CM. A piece of coal/rock fell out from the roof and struck employee on the back & neck. Employee finished installing the bolt & stepped away from the bolter station toward the rear of the CM. Another piece of coal fell from the roof, hit the rock deflector & then hit employee on the back as they were walking away.

Injured employee positioned a longwall leg jack onto the bedplate extension so the jack could be removed from the face. EE failed to secure the jack against movement. Employee briefly looked away from the jack & at that time the movement of the shearer caused the jack to roll off the bedplate onto EE's left leg resulting in a fracture to the fibula.

Employee was using a grinder to cut on a belt cradle on the 4 West #2 Belt. When the grinding wheel made contact with the cradle, it "kicked" back & contacted employees left leg causing a laceration above EE's knee. The laceration required 4 sutures to close. Employee failed to maintain control of the grinder during use. Employee was wearing required PPE.

Employee was installing roof bolts from the right side of the 5W continuous miner. EE was holding a drill steel in the bolter head to guide it in place, they inadvertently pushed the tilt lever instead of the up lever. This caused them to be pushed backward & they hit their back against the machine's rib protector. Employee has training & experience. Began missing work on 1/12/23.

Employee was installing roof bolts from the left side of the Continuous Miner on the 5W Section. As EE was pushing the top half of a combination bolt up into the bolt hole EE just drilled, EE felt pain in the right shoulder. Employee has training and experience. No rules or regulations were violated.

Two employees were rockdusting on the 4 West #1 Belt. One employee was holding the pressurized rockdust hose when the tripped and lost balance causing them to lose control of the dust hose. The hose sprayed rockdust into the other employee's eyes. Injured employee received scratches to their right eye. Employee didn't violate any rules or regulations and has training & experience.

Employee was roof bolting on the left side of the 5W continuous miner when a piece of coal/rock fell out from the upper part of the rib behind them and struck them in the back of lower leg. Has bruising, pain and swelling. Employee began missing work on 6-24-22 due to this injury. Employee didn't violate any rules or regulations and has training and experience in task.

Employee was working at the longwall belt booster drive. A piece of PVC pipe that was cut out of the longwall face, traveled on the belt to the transfer where it jammed and came off the belt hitting employee in the mouth. Employee had to have a tooth removed as a result. No rules or regulations were violated. Employee has training & experience.

The injured employee was trouble shooting an oil pump. When EE turned the air supply on the pump, plunger piston moved and contacted EE's right thumb resulting in a laceration requiring 7 sutures.

EE was holding a drill steel in place with EE's right hand and was attempting to press the lever to dump EE's dust collection box but inadvertently pressed the tilt lever causing EE's finger to be smashed between the drill steel and strap jack.

Employee was installing Tensar roof screen at the face of the 4W CM section during mining. As employee was raising roof jack against the screen, right pinky finger got pinched by the screen causing a small avulsion/laceration to the tip of the finger. Employee failed to pay attention to hand placement.

Injured employee was installing a rib bolt from the left side of the continuous mining machine. IE was attempting to guide the scroll style drill steel into position as IE pushed the feed lever; however IE pushed the rotation lever instead. The steel rotated and cut IE's left index finger. IE failed to pay attention to the levers IE was pushing. Received 5 sutures.
