IE was installing a roof bolt. As the employee went to push the bolt that EE had bent into the roof with the drill pot of the roof bolter the bolt sprung back and struck EE's left forearm.
IE was operating the shearer when a small rock came from the drum and hit EE's cheek before glancing into EE's eye and causing a laceration.
The conditions was hot, damp and clean. The injury was to the left foot at the toe, the damage was a miner cut and bruising followed by increased pain and swelling later that day.

EMPLOYEE WAS STICKING ANOTHER STEEL IN THE CHUCK WITH LEFT HAND WHEN EE ACCIDENTALLY HIT THE ROTATION LEVER WITH RIGHT ELBOW. EE HAD A HOLD OF THE STEEL TOO HARD, AND WHEN EE HIT THE ROTATION LEVER WITH THEIR ELBOW, IT SPUN QUICKLY AND POPPED EE'S SHOULDER OUT.
IE was roof bolting on a CM section. IE was pushing the roof bolt to the roof when the bolt bent and caused excess strain on the wrench. The wrench broke and when the employee put hand up, got a laceration between the thumb and index finger.
IE was operating the shearer when a rock was thrown from the drum and struck the employee on the leg.
IE was completing a pre-op on a tractor when the dust hose that was attached to the rock duster blew off. The hose knocked the employee down and landed on EE's shoulder.
IE was setting bits on the shearer when a piece of draw rock fell and slid down their neck to their shoulders. As the employee stood up a piece of draw rock fell striking their left foot.
IE was removing a leaf spring on a truck when IE's wrench slipped. IE's hand contacted the spinning nut causing a laceration.
IE was operating the shearer cutting out the tailgate when a rock came from the drum and struck IE's left ring finger against the pump compartment on the shearer.

IE was standing under a cable for a roof bolter when the bolter moved. The roof bolter turned the reel on and it pulled the cable down on the employee. The employee worked multiple shifts before requesting treatment.
IE was roof bolting when the drill steel got caught in the roof. As the employee was lining the drill steel up to the pot they bumped the rotation. This caught the employee's glove and dislocated two fingers.
IE was roof bolting on the 3-intake seal rehab when IE dropped a drill steel from the roof catching a finger between the pot and the drill steel. This caused a laceration to the left ring finger.
IE was washing at the slope tail when IE let the hose get away. In trying to catch the hose it hit IE multiple times and knocked IE into the rib.
IE was installing a 3-foot roof strap when the strap rotated striking the employee above the right eye. The IE was taken to the hospital and received stitches.
IE was operating the shearer when the IE let the drum hit a roof bolt plate in the service entry. The bolt plate came and hit the employee in the right forearm causing a laceration. The employee was taken to seek medical attention.
IE was using a grinder with a wire wheel on it. The employee dropped the grinder causing it to kick. The wire wheel caused some scratches and cuts to the employees forearm and 2 fingers. The employee was taken to seek medical treatment.
Crew was mucking. Muck Bucket hit employee in head due to brakes catching, because the hoist was bouncing.
IE was drilling with a breast auger when the drill caught and twisted striking EE's arm.
IE was running the shearer when a rock was thrown from the drum and struck EE in the leg. EE finished the shift and wanted to seek medical attention the next day.
IE was roof bolting when EE's chuck came out of EE's drill pot. The IE did not secure the drill steel in the mine roof before picking up the chuck. The drill steel dropped from the top and hit the employees left ring finger. EE was brought to the surface and taken to be checked out.
IE was drilling shot holes when IE had to add the pusher steel. IE did not support the starter steel and it fell from the hole IE was drilling and struck the tip IE's right index finger. This caught the finger between the drill steel and the drill pot. IE was brought outside and taken to have the finger checked.
Employee was securing a monorail winch to the mine floor when 1 of the monorail chains above the winch broke allowing the winch to fall. The IE was under the winch at this point. The winch was picked up and the employee was brought to the surface and transported to the hospital. MSHA was notified via the hotline and MSHA investigated the incident.
IE was installing mesh on the roof. IE had to lower EE's canopy of EE's roof bolter to allow clearance for the mesh. IE had hand EE's left hand too close to the canopy jack and caught EE's left pinky finger between the jack collar and the canopy post causing a laceration. IE was brought and taken to be checked out.
IE was roof bolting when IE's drill steel got stuck in the mine roof. In the process of trying to free the drill steel IE dropped it hitting right pinky finger. IE was brought out and took to be checked out.
