REMOVING DISCHARGE VALVE COVER FOR MAINTENANCE. VENT VALVE WAS CLOSED CAUSING THE PORT TO BE PRESSURIZED. WHEN HE REMOVED ALL OF T HE BOLTS FROM THE VALVE COVER BLEW OFF & HIT HIS LEFT HAND AGAINST THE CONCRETE FLOOR.

INSPECTING MAGNETIC SEPARATOR WHICH SEPARATES METALLIC WASTE. SEPARATOR EJECTED PIECE OF METALLIC WASTE WHICH HIT INJURED\S LEFT HAND INSTEAD FO GOING INTO THE BIN ASSIT IS SUPPOSED TO. SINCE THE INJURY GUARDS HAVE BEEN INSTALLED FORWARD ANDON BOTH SIDES OF THE SEPARATOR.
