Employee failed to unplug chop saw prior to changing gout blade. While holding wrench on nut, they activated the saw which twisted wrist back. Injured saw doctor for strain but lost no time. On 3/10/2021 injured is now going to physical therapy but still able to work with restrictions.

Showing all 5Injured was struck by 4" dia. x 4'long pipe as it was being suspended with crane. Chain rigging slid off pipe about 6' from ground. Pipe hit structure before striking Injured on Left foot. Injured was wearing steel toe boots. Need to be on crutches for two weeks for minor bone fracture to top of foot.
Employee was greasing bearings with pneumatic grease gun when ee accidently shot chest with gun while putting gun away. Abrasion to chest wall from impact. Employee was released back to work with no loss time. Additional visits were later needed due to slow healing of wound. Employee is diabetic and restrictions are keep area clean and dry.
The hydraulic hose on boom truck crane malfunctioned causing the tire on the crane to swing and strike employee on the right leg/knee area.
INJURED WAS ADDING PIPE COUPLING TO DREDGE DISCHARGE PIPE. COUPLING SLIPPED AND SHARP EDGE CUT RIGHT FINGER ON HAND.
