EE was loading injured employee's truck with gummy overburden. While dumping bucket the material came out in a wad and injured EE said that it hurt their neck. EE removed from duty and sent for evaluation at their request. Diagnosis strain of trapezius, given medication and able to work with restrictions.

Showing all 32On Thursday at about 4:00p EE was operating an articulated truck in the mine area. EE was parked in position for loading then became jolted in the cab by an excavator bucket being operated by co-worker. EE called supervisor at 7:00pm on 11-1-19 to report neck discomfort and medical assistance. EE got medical treatment on 11-2-19. Possible strain of neck muscle.
Employee opened a door, reached into equipment with right hand & mill rollers grabbed glove pulling hand into mill damaging multiple fingers. EE was transported to Emergency Room. Three fingers were amputated and one finger was sutured.
Assumption is that EE lifted latches that secure a door, opened the door & put left hand inside mill (mill 02) while the mill was in operation. As of the date of this report the EE has declined to provide a statement of the incident & there were no witnesses. Employee suffered amputation to left thumb and 2 or more fingers.
while working with a jackhammer, came in contact with rebar and cut finger
EE was removing paper on pulley, belts running. Belt caught glove pulling it between 2 pulleys causing a laceration & hair line fracture to left hand 2nd finger tip. Required sutures & restrictions of limited use of left hand until 3-4-13 then, keep dressing clean & dry.
Left hand placed on machine pinch point & EE's 4th finger was pinched. Caused scrape to his knuckle & small break on underside of finger. Initially doctor applied bandage only (1st aid) & on a 9-14 follow-up visit restricted him with no use of left hand.
Pushed emergency stop to clear jam,then pushed e stop & restart. Had hand on machine & tape head came forward pinching right middle finger. Partial amputation & skin graft of right middle finger. Restricted no lost time.
EE was using dozer to clear vegetation when dozer ran over a tree stump causing dozer to be jarred quickly & awkwardly. EE hit head on left side of cab causing a laceration on top of head. Required sutures, but no restrictions.
EE in route to work on OMW Line 2. While walking by Line 4 fans a foreign body went into his left eye. EE was wearing safety glasses. Sent to doctor and diagnosis was abrasion to left eye. EE was given prescription and no restrictions.
He was modifying an aluminum basket with a grinder. He installed a cutting disk & began cutting when his left hand slipped off the handle of grinder cutting his left thumb & left index finger. Had surgery to repair laceration to tendons for left thumb & index finger. No lost time just restricted.
Trouble shooting Palletizer 1 & pallet jammed in empty pallet dispenser. Employee flagged photo eye with hand & activated pallet dispenser catching his left hand between pallet & mount. Fractured bone from left index finger to wrist. Wearing soft cast & restricted use of left hand.
Adjusted lower limit switch for carriage travel on Lantech Wrapper. Was moving carriage into home position and tension roller caught hand pulling it into frame of carriage cutting his right hand pinky & ring fingers. Required sutures. No lost time, just restricted.
Was placing bag in tilt chamber, chamber cycled & finger was caught between rake & rolled edge of tilt chamber. Amputation of first joint, ring finger, left hand. He has restricted use of left hand. No lost time.
Employee was working on the bag kicker when he ran his finger across the limit switch. The air was not released and the kicker moved back catching his left index finger between the kicker and the bracket. He received a laceration to his left index finger requiring sutures. He has restrictions and is on light duty for 14 days.
She was trying to unclog suction cup on Bemis Bagger. She reached her left hand in to remove green filter from suction cup while machine was still running. Her wrist was squeezed causing a bruise to her wrist.
PALLET WAS JAMMED, WERE UNJAMMING THE PALLET AND THE PHOTO-EYE ACTIVATED THE SQUARING CYLINDER. EE'S FINGER GOT CAUGHT BETWEEN PUSH PLATE AND PALLET.
BAGS WERE TOO FULL AT THE TIME AND EE WAS TURNING TO SPEAK WITH THE DUMPER OPERATOR AND WAS NOT WATCHING WHEN HIS THUMB ENTERED THE SEWING MACHINE IN A FEED.
Employee pinched off 3/8 inch off tips of middle, index, and pinky finger. Employee violated company's lock out tagout policy and reached hand into rotary valve that was in operation. Contact with valve caused injury.
BLOWING OFF TRASH AND SOMETHING FLEW IN HIS EYE.
EE SAW BAG CLAMP OPEN. HE STOPPED MACHINE AND OPENED DOORS. HE REACHED FOR CLAMP ROD. THE CLAMP RELEASED TENSION AND CLOSED, CATCHING HIS FINGER IN BETWEEN.
THE EE WAS USING AN ACETYLENE TORCH TO CUT A PIECE OF SHEET METAL. WHILE CUTTING THROUGH THE METAL, PIECES OF MELTING SLAG FELL ON HIS LOWER LEG CAUSING HIS COVERALLS TO CATCH FIRE. THE FIRE WAS QUICKLY EXTINGUISHED BY THE EE AND HE WAS TAKEN FOR MEDICAL TREATMENT.
EE WAS ATTEMPTING TO PREVENT A 10 LB BAG FROM FALLING ON THE CONVEYOR ONT HE BAG LINE. HE ACCIDENTALLY STUCK HIS HAND BEHIND THE SIDE CONVEYOR AND HIS HAND WAS PULLED INTO THE HEAD PULLEY WHER E IT BECAME STUCK.
THE EE WAS ATTEMPTING TO REMOVE THE BELTS FROM AGLUE CLOSER MACHINE USING HIS HAND & BY TURNING IT OFF & ON WITH FREE HAND. AS THE MOTOR ACTUATED, HIS HAND WAS PULLED INTO THE PULLEYS & ROLLER S RESULTING IN A CUT ON HIS RIGHT INDEX FINGER.
OPERATOR WAS REPLACING TWO BALES OF CAT LITTER THAT HAD FALLEN FROM A PALLET. THE REPLACEMENT WAS CONDUCTED IN THE PATH OF A TRACK GUIDED FINISHED PRODUCT TROLLEY TRANSPORTING PALLETIZED MATER IAL TO BE STRETCH WRAPPED. THE OPERATOR FAILED TO ACKNOWLEDGE THE SAFTY LIGHTS AND MOTION ALARMSAS PALLETIZER MOVED DOWN THE TRACKS AND WAS STRUCK ON THE RIGHT LEG BY THE PALLETIZER.
THE OPERATOR WAS RESTACKING A BALE OF CAT LITTER BACK ON A PALLET IT HAD FALLEN OFF ON LINE #2. WHILE RESTACKING THE BALE HE WAS STANDING IN THE PATH OF A TRACK GUIDED FINSH PRODUCT PALLET TRO LLEY THAT TRANSPORTS PALLETIZED MATERIAL TO THE STRETCH WRAP MACHINE. THE OPERATOR WAS BUMPED BY THE PALLETIZER CREATING A CONTUSION ON HIS LOWER RIGHT LEG. THE PALLETIZER IS EQUIPPED WITH NU
EE WAS GRINDING METAL INSIDE OF A DUMPSTER WHEN A PIECE OF METAL BOUNCED OFF A WALL AND WENT INTO HIS EYE. HE WAS TAKEN TO THE CO. DR. WHERE THE METAL WAS REMOVED FROM HIS EYE. HE WAS WEARING A SAFETY SHIELD AND SAFETY GLASSES AT THE TIME OF THE ACCIDENT.
SUPERVISING EMPLOYEE GRINDING METAL; PIECE OF METAL FLEW INTO HIS LEFT EYE.
EE PULLED LEVER ON BACKHOE AND PAIN SHOT DOWN SHOULDER AND RIGHT ARM. PAIN WORSENED AS DAY PROGRESSED.
EMPLOYEE WAS GRINDING THE EDGES OFF OF A PIECE OF METAL USING A BENCH GRINDER. THE GRINDING ROCKGRABBED THE METAL AND HIS THUMB INTO THE GRINDING CAUSING A DEEP CUT. THE EMPLOYEE WAS REQUIRED TO BE HOSPITALIZED OVER NIGHT AND HIS THUMB WAS SUTURED TO CLOSE WOUND.
EE WAS MANUALLY LOADING BAGS INTO THE BAG SPACER SECTION OF THE VALVE PACKER MACHINE BECAUSE THE CLAMP WOULD NOT AUTOMATICALLY PICK UP THE BAGS. WHILE PUSHING THE BAGS THROUGH BY HAND, THE MAC HINE ACTUATED AND JAMMED HIS RIGHT WRIST CAUSINGA CONTUSION. HE RECEIVED MEDICAL ATTENTION AND WAS RELEASED TO RESTRICTED DUTY.
EE WAS ATTEMPTING TO PULL A BAG OF CAT LITTER FROM A DEVICE CALLED A TILT CHAMBER. WHILE REMOVING THE BAG, THE DELIVERY ARM ON THE DEVICE STRUCK HIS LEFT FOREARM CAUSING A HEMATOMA. THE INJURY REQUIRED THAT HE WEAR A SPLINT.
