Showing all 5Miners were adjusting the timing of gates on the Lambert Palletizer Model 1200. Miners must be on equipment while in operation to make these adjustments. The guard had been removed from the left gate. The effected miner's left foot was pinned between the gate and the shaft when the gates were opened.
Employee was exposed to welding flash when working on outside of sand screw tub while maintenance welders were working inside tub. The employee received welding flash burn injury to to eyes requiring employee to miss one day.
