THE ACCIDENT OCCURRED AT THE START OF THE SHIFT WHEN AN EMPLOYEE WAS HOISTED IN A CAGE BY A HYDRAULIC CRANE TO CHANGE A YARD LIGHT ATOP A 30 FT. POLE. AS THE CAGE WAS HOISTED, THE HOOK BLO CK WAS BROUGHT VERY CLOSE TO THE SHEAVE ON THE END OF THE BOOM. IN AN EFFORT TO BETTER POSITION THE CAGE, THE BOOM WAS EXTENDED WITHOUT ALLOWING ANY SLACK IN THE CABLE CAUSING THE HOOK BLOCK
