INJURED WAS GREASING A BEARING NEXT TO AN EXPOSED SHAFT, HE LAID HIS ARM ON THE SHAFT WHICH CAUGHT HIS SHIRT SLEEVE AND SPUN HIM AROUND THE SHAFT SEVERING HIS LEFT ARM, BREAKING HIS THREE RIBS PROXIMAL TO THE ARM, AND CAUSING A SKULL FRACTURE AND SEVEREAL FACIAL LACERATIONS. HIS ARM WAS AMPUTATED AT THE SHOULDER AND THE OTHER INJURIES TREATED.
