2 PTCs were doing their routine afternoon cleaning of the 4th floor riser. EE had the NE top door open to start cleaning when the riser went positive for approx. 5-10 seconds. EE could not get out of the way quickly enough before hot gases slightly burned the right side of EE's neck.

Employee received laceration on lower right leg from high pressure water lance that required 6 stitches.
Employee was welding in a restricted area that required you to light the torch prior to reaching work area due to lack of room. As the employee was accessing the work area the touch hose got hung up on a thermo coupler on the floor causing the hot torch to go across EE's hand.
Employee dropped a pneumatic jackhammer on finger tip and cut the end of finger. 2 stitches required.
Emp was cutting a piece of steel with Iron Worker. While cutting piece it folded under cutting emp Lt. hand index fingertip between the steel and the framework of machine. Finger was bandaged and no stitches / No lost time/restrictions for five (5) days to keep cut clean.
Emp was clearing out a clay hopper with an air lance when EE struck a hand rail with EE's Lt. hand middle finger. Emp was wearing gloves. Emp pinched the tip of EE's finger between lance valve and rail causing a fractured tip. Lt. Hand middle finger fracture / splint.
Employee was using a water blasting gun to clean riser duct of kiln. EE turned head just as a puff of dust came out of duct causing foreign material to get into Lt. eye. Sent to ER and examination showed abrasion of eye / Prescription Eye drops were used. No lost time came back on next scheduled day.
THE EMPLOYEE WAS CUTTING WITH A TORCH ON THE HOPPERS WHEN A SPARK FLEW UP UNDER WELDING HOOD AND SAFETY GLASSES HITTING EE IN THE RIGHT EYE. EE FLUSHED EYE OUT AT A WASH STATION & THOUGHT THAT, EVERYTHING WAS OUT OF EYE. ON 04/26/16 EYE CONDITION HAD WORSENED THO.
Contractor MSHA I.D.#S146 All-Around employee was using a cutting torch when something flew in Rt. eye under welding hood and safety glasses. EE did not report injury until 04/26/2016. Object was embedded.
Emp was welding, grinding and burning. At some point a piece of metal entered his Rt. eye. Emp thought he flushed it out, but his eye was bother him when he returned on Monday, 03/17/2014. He reported it and was sent to eye specialist. A piece of metal was imbedded in the cornea of eye.
Emp was operating dozer when he ran over a large rock jarring his Lt. arm. Emp continued full duty work until he complained of discomfort and was sent to Occupational Health were he received restrictions. No operating dozer on 3/21/2014.
Emp. was cleaning out PreHeat Tower vessel with high pressure water blaster gun. Lrg. chunk of matl. fell striking gun causing emp. to fall backwards, pulling gun out of vessel as emp. fell. Trigger struck on gun causing it to go out of control/high pressure water struck emp. in lower leg. Laceration/stitches.
Employee was welding, cutting, and grinding when his Rt. eye started to feel painful and swollen. Employee stated he thought he might have a welding flash. He was sent to eye specialist for examination and a piece of steel was embedded and removed from his Rt. eye.
Emp was working on pantleg of clinker area pan conveyor. Torch was ignited in kiln area causing a blast of air/dust to travel down pipe to opening in pantleg where emp was working causing emp to move backwards striking his neck on pipe. This occurred on 10/17/2007 emp has been in PT but continued discomfort has required surgery to Rt. shoulder 08/04/2008
Employee starting the high pressure water gun to clean the PreHeat Tower. Due to the sub-zero weather gun may have had a piece of ice in hose/employee reached to turn valve when the gun came on causing water stream to strike employee's Lt. Ankle cutting his boot top and lacerating his ankle. Employee has been hospitalized to receive intravenous antibiotics to prevent infection.
The IW was demoing a screw conveyor. The IW had his hands placed on top of the conveyor. Just as the conveyor freed, the IW placed his rt hand on the torch-cut end. The conveyor thus swung towards the torch-cut end smashing the IW's hand between it and some steel supports. The IW received lacerations to the rt hand at the base of the index and middle finger.
Employee was bent over grinding a 1/2" steel plate when he felt a sharp pain in his lower back. Strained back after being bent over in an awkward position.
EE WAS OPERATING AN AIR DRIVEN BARREL PUMP. THE DISCHARGE HOSE BLEW OFF THE PUMP LACERATING HIS HAND.
