Showing all 28An employee was WOMA Gunning the tower on the 2.1 level when a chunk of material fell on the wand and caused the employee's right arm to be jerked. Employee reported to supervisor, but did not want any medical treatment at the time of reporting the incident. On 3/13/2025, employee still felt soreness in their right shoulder and requested to be evaluated. Had surgery on 6/9/25
While removing a drive sprocket with a porta-power and gear puller, a piece of the sprocket came free and struck the EE on the edge of EE's lower neck which caused a small laceration that required a stitch.
While drilling through a piece of airslide fabric, the EE placed his finger on the backside of the fabric and contacted the drill bit.
Employee was performing routine cleaning in the Preheat Tower with a WOMA gun. When the employee initiated the WOMA gun, hot feed blew back onto the employee causing burns to the back of his unprotected head, neck, and left ear. EE refused to wear PPE.
Employee was burning off a bolt. A piece of hot metal caused 2nd degree burn to palm of his hand.
Supervisor was attempting to unplug blockage on 3rd stage of pre-heater tower using an air lance. Hot material back flushed causing 2nd degree burns on his forearms and back of head.
Employee was attempting to unplug a kiln seal hopper. He was using a chipping gun and must have loosened dust from an eye beam. When he took off his goggles, a small amount of dust got in his left eye.
While doing demo work, the employee was cutting an 8" I-beam. The rigging slipped, the employee's glove was caught by the I-beam as it was falling, pinning his hand. Laceration on right ring finger required stitches.
This was a first-aid case. Physician called today and stated that employee has a torn meniscus tear to right knee - caused by water lance nozzle hitting his knee
Employee was assisting in moving a barge. He was preparing to attach the pull chain to the barge cleat. The cable sheave broke allowing the cable to fall. This jerked his arm causing a fracture to his left forearm.
Injured employee was a stand-by person on a high pressure water blasting task. The person controlling the water blaster tripped and fell backward. As he was falling, he accidentally hit the injured employee with the high pressure water, causing a puncture/laceration wound to the employee's right forearm and right side of chest.
The EE was using a high pressure sprayer. The EE was spraying inside riser. Inside the riser a chunk of material fell striking the end of the sprayer inside the port. This sudden action jerked his shoulders. At the time of the occurrence the EE did not wish to seek medical treatment. He was sent to see the doctor on 7/16/04 and placed on modified duty.
THE EE WAS ASSISTING A WELDER WHILE INSTALLING A METAL PLATE AS A COVER OVER A HOLE. THE EE WAS HOLDING THE METAL PLATE IN PLACE WHILE THE WELDER TACKED IT. WHILE DOING THIS THE EE DID NOT SHIELD HIS EYES SUFFICIENTLY AND RECEIVED A FLASH FROM THE WELDING ARC.
EE (1) WAS HANDING A PRESSURE WASHER GUN FROM PLATFORM C TO PLATFORM D WHEN ANOTHE EE (2) ACCIDENTALLY PRESSED THE GUN TRIGGER WHILE ATTEMPTING TO GRASP THE GUN. THE GUN DISCHARGE AND HIT EE ( 1) IN THE LEFT INNER THIGH CAUSING A CUT.
EMPLOYEE WAS GRINDING ON DOOR OPENING REAR OF TRIPLE GATE. GRINDER CAUGHT ON ROUGH EDGE OF METAL, SLIPPED OUT OF HIS HAND, STRIKING HIM IN THE LEG, LOWER THIGH.
EE WAS USING A WOMA GUN (HIGH PRESSURE WATER SYSTEM) WHEN IT CAME APART DUE TO A DEFECTIVE COUPLING. HIT HIS LEFT HAND CAUSING A LACERATION AND SWELLING.
EMPLOYEE WAS ATTEMPTING TO WOMA GUN IN A PORT THAT HAD MATERIAL PLUGGED IN IT. WHEN HE SQUEEZED THE TRIGGER THE WATER HIT THE HOT MATERIAL AND HOT DUST AND STEAM CAME BACK TOWARDS THE EMPLOYE E. THE MATERIAL WENT INTO HIS GLOVE AND UP HIS SHIRT SLEEVE. EMPLOYEE DID NOT MISS TIME UNTIL 5-15-02.
EMPLOYEE HURT KNEE WHILE OPERATING THE DOZER WHEN HE PUSHED AWKWARDLY ON THE BRAKE PEDAL. EMPLOYEE DID NOT START MISSING TIME UNTIL 7/30/01.
WELDING ELECTRODE STUCK TO STEEL, HE JERKED IT LOOSE IT CAME UP AND HIT HIS TOOTH.

EE WAS WELDING A PIECE OF ANGLE IRON ON A PLANT FORM TRACTOR. THE ANGLE IRON FELL, CRUSHING THE EE'S FINGERTIP BETWEEN IT AND THE TRACTOR.
EMPLOYEE WAS GRINDING WELDS ON TOP OF DUCT. A WELDER WORKING IN CLOSE VICINITY STRUCK AN ARC CAUSING A FLASH.
CRANE WAS LOWERING STEEL INTO PLACE; HE SPUN THE BEAM AROUND & GRABBED THE CLIP. THIS STEEL WAS A PIECE THAT HAD TO BE ERECTED AMONG PREVIOUSLY ERECTED STEEL, DUE TO LATE DELIVERY. AS BEAM S PUN AROUND, THE CLIP CAUGHT ANOTHER PIECE OF STEEL, RESULTING IN INJURY TO LEFT THUMB.
EE WAS FITTING PLATE TOGETHER TO BE WELDED. WELDER STRUCK AN ARC, CAUSING A FLASH. RECEIVED FLASH BURN TO EYES.
GRINDING WELDS ON HOPPER PLATES. FOREIGN OBJECT ENTERED EYE.
EMPLOYEE WAS HANGING STEEL AT THE PREHEATER WHEN HIS THUMB BECAME TRAPPED BETWEEN THE STEEL BEAM AND THE CONCRETE WALL.
