EMPLOYEE WILFULLY CIRCUMVENTED A GUARD BY KNEELING ON THE GROUND, REACHING UNDER THE GUARD AND STICKING HIS FINGERS THROUGH A NARROW OPENING 1 1/2" THE EMPLOYEE STATED INTENT WAS TO FEEL THE B ELT TO SEE IT THE BELT WAS RUNNING RATHER THAN WALK A SHORT DISTANCE TO GET A FLASH LIGHT. THE V-BELTS CAUGHT HIS FINGERS AMPUTATING THE FIRST AND SECOND FINGERS AND LACERATING THE THIRD
