ONE LENGTH OF STEEL HADJUST BEEN REMOVED AND THE DRIVE HEAD OF THE DRILL WAS BEING LOWERED TO CONNECT TO THE REMAINING LENGTH OF STEE5. THE EMPLOYEE NOTICED THAT THE WEDGE PLATE WAS LOOSE AND USED HIS FOOT TO PUSH THE PLATE BACK INTO PLACE. THE DRIVE COLLAR CAUGHT THE STEEL TOE OF THE EMPLOYEE'S BOOT BETWEEN
