During preparation for dismantling of motor and gearbox on slide gate EE's coworker released all bolts on gearbox which cause spinning of motor and gearbox. Coworker got caught by equipment and started spinning with it. During spinning coworker's body hit employee and caused them bruises on their leg and arm.

Showing all 23Employee was water blasting in the tower. As EE was holding the gun, EE pulled the hose causing the safety pedal to be pushed down. This in turn pressurized the gun causing it to strike the miner above the left eye.
Expanded metal slid and cut EE's right forearm. Accident time unknown safety manager on vacation.
While grinding, grinder became wedged between flapper gate causing the grinder to kick back and strike employee in area above the right knee. This caused a 6 cm laceration to that area. This resulted in 7 stitches

Employee was operating D-9 Dozer in Quarry. He complained of soreness in the area of his tail-bone. Apparently employee developed a boil in tail bone area possibly due to bouncing of dozer in rough terrain. Investigation of seat in dozer indicated that the seat was in good condition.
Employee removed a safety lock and turned on the air on, an air blaster that had the discharge end disconnected. The blast of air struck the left arm and caused an open wound.
Employee was installing a hydraulic portable jack below the socket in the roller mill when the ram slipped and fell on the finger tip causing a fracture.
Employee was using an air lance to clean cement silo #1. Employee did not think ear protection was necessary and injured the ear drum.
EE lost balance while jack hammering on 62BC11 and fell against a support beam about 3-4' away from his position, bruised right hip
EE had removed a cardox tube from the base socket attached to the meal pipe in the preheater tower. Cardox shot was successful in unplugging a blockage & the flow of material resumed shortly therafter. Hot dust discharged through the base socket, striking EE & causing burns to both forearms, wrists, R thumb, L little finger, R ear.
While aligning the ball mill manway door, overhead hoist cable failed, causing the line, hook block assembly to strike the employee on the head/neck area.
Individual was preparing to cut a bolt with a torch. He reached up to place his cutting goggles over his safety glasses. When he did debris fell into his eyes and face.
PRE-HEATER PLUGGED IN AREA OF CY-5. EE FULLY OUTFITTED IN HEAT PROTECTIVE WEAR DRILLED HOLE THROUGH SHELL WALL INTO CYCLONE. WHEN HOLE BREACHED BRICK INTO MATERIAL - HOT MATERIAL PUSHED OUT TH ROUGH HOLE - HOT MATERIAL PUSHED UNDER PROTECTIVE GLOVE AND BURNED THROUGH INNER PROTECTIVE ARM GUARD.
EMPLOYEE OPENED PORT ON TOWER TO CHECK FOR PLUG - HOT AIR AND DUST BLEW OUT WHEN "AIR BLASTER" DISCHARGED HOT DUST BLEW ONTO ARM AND SIDE OF FACE AND HEAD.
EMPLOYEE WAS USING A 1-1/2" IMPACT WRENCH FOR MOST OF HIS SHIFT. WHEN THE SHIFT ENDED HE NOTICED PAIN IN HIS RIGHT HAND EXTENDING UP TO THE TRI-CEP AREA.
ANOTHER EMPLOYEE WAS HEATING A BRACKET SO THAT IT COULD BE BENT SLIGHTLY. INJURED WAS SUPPORTING BRACKET - USING A "C" CLAMP. THE "C" CLAMP SLIPPED - BOTH EMPLOYEES MOVED WHEN THIS OCCURRED. I NJURED MOVED HIS GLOVED HAND ACROSS THE FLAME OF THE TORCH - THE GLOVE CAUGHT ON FIRE.
EE WAS JACKHAMMERING AND SMASHED HIS LEFT THUMB AGAINST CLAY BAILEY.
THE CREW WAS PUTTING BURNER PIPE IN PLACE AND BURNER PIPE FELL 8 FEET STRIKING EMPLOYEE.
