Showing all 5Working with an impact socket, the "knuckle" came loose and struck his left pinky finger several times at the joint just below the fingernail.
Was cutting in between two decks on screen. Hot slag from cutting bounced onto side of screen and bounced into his ear. He did not report the accident until 12/26/07. He was sent to urgent care to be checked. Urgent care Dr made appt for him on 12/28/07 with an ENT. He was checked and given antibiotics. NO LOST TIME.
Welding rod stuck between 2 tires, couldn't reach it used foot to kick it.Burned thru rt boot.He kept area clean, used antiseptic.7/26 it became red & swollen went to Urgent Care.Diagnose w/quarter-size burn/puncture between big & 2nd toe w/cellulitis.Wound debrided, given antibiotic & Slivadene burn cream-told to clean twice/day.Allowed to return to work w/out restrictions.
VISIT PHYSICIAN EE WAS WELDING ON PRIMARY A HOT PIECE OF METAL FELL DOWN INTO HIS BOOT AND CAUGHT SOCK ON FIRE NO LOSS TIME. NO CAUPLICANCE RULES INVOLVED. NO DISABILITY.
