INMATE WAS REPLACING BELT ON ROCK CRUSHER MACHINE WHILE MACHINE WAS IN ACTIVE OPERATION, USING SCREWDRIVER. SCREWDRIVER HAD GOT CAUGHT INT HE PULLY, SLIDING CROSSWAYS, CUTTING/RIPPING END OF 3RD FINGER FROM THUMB OF THE RIGHT HAND.
