Using a vac hose, the suction pulled EE forward strain/pulled EE's lower back. Initially declined medical attention. Pain increased and requested medical on 02/12/2025, which were able to schedule. Restrictions were placed with that appointment.

Closing a clay belly door on airslide when it went positive and blew cement dust out and went into eyes. Employee was wearing safety glasses it was just an unexpected plume of material.

Cleaning out preheater, was near an open port and material "burped". Employee had backed away and opened visor on Helmet which allowed material in right eye.

Employee was replacing a lid for a chamber in the main dust collector penthouse. Lid was being replaced using overhead crane. Lid needed approximately 2" adjustment to slid into place and when adjustment was made the lid quickly popped into place and employee had finger between lid and frame. Finger was smashed in between lid and frame causing injury to right hand middle finger.

While making a blind lift, with 3 flagmen, the tip of a mobile crane bumped an overhead beam, dislodging buildup, that fell and contacted the left hand of the repairman rigging the lift. Employee suffered fracture of left hand. Employee did not require surgery, and is in a cast, on restricted duty.

While installing brick in a kiln a shim driver was used to help install shims for the new install. The shim driver jumped off of the shim, hitting the employees left, pinky finger.

Using an End Grinder while wearing safety glasses and face shield, a gust of wind come up causing some debris to kick up and get into EE's eye.

Employee was in a scissor lift and was cutting a piece off of the cable tray with a cordless porta band. Employee was holding the material with EE's left hand when it slipped and cut two fingers on left hand.

Miner was using a 6" grinder. Grinding wheel struck left index finger. Miner went to clinic, was examined, and given a tetanus shot. Was referred to emergency room for further treatment. After examination and negative X-Rays the physician cleaned the cut, applied ointment, bandaged, and applied a metal finger guard. Anti-biotic pills were prescribed.

Employee was troubleshooting instrumentation on kiln, as they broke the union loose, an airblaster cycled blowing their glasses off, and depositing dust in their eyes. Physician cleaned out eyes, and prescribed medication.

Employee had been operating dozer, using ripper on uneven and hard material. Over the course of 2 days, employee began to have discomfort in neck. Physician prescribed no restrictions, until surgery was performed 10/16/18. Expected a few lost workdays before returning on restricted duty.

Employee was jack hammering buildup in cement elevator, when ee felt strain in lower abdomen. EE did not initially report, but as discomfort persisted, reported on 8/4/17. Physician diagnosed as thrombosed hemorrhoid. Procedure to drain resulted in restricted duty.

Pressure was applied to roller mill hydraulic system to check for leaks. A leak was detected at fitting, which was tightened to stop leak. The fitting broke loose while tightening, and struck EE in the ribs. Physician diagnosed as contusion, prescribed limited lifting etc.

EE TURNED OFF BREAKER TO THE S BLOWER, DID NOT PLAN TO WORK ON BLOWER, SO DID NOT LOCK OUT. THEN WENT TO CHECK INFO ON N BLOWER, EE REMOVED GUARD, AND LEANED OVER TO GATHER THE TAG INFO ON THE MOTOR. RIGHT HAND CONTACTED BELTS, AND THE BLOWER STARTED AS THE SPOUT BEGAN LOADING A TRUCK. FINGERS WERE PULLED BETWEEN BELTS & PULLEY, 2 FINGERS WERE CUT.

Injured Employee was Asst. Plant Manager, supervising an employee using an air lance to clean material behind mill rolls. The small wire had been used to secure the fitting became unsecured and the fitting struck the ee in the mouth. Dentist prescribed antibiotic for lower lip injury.

EE was welding inside crusher. After returning to work the following day, EE reported that his eyes were "burned" from reflected glare of welding. EE was wearing welding hood. Subsequently, EE realized there was a small metal shaving in eye. Physician removed shaving.

The employee was moving an overhead 5-ton hoist by pulling the chain. He felt a strain in his right shoulder. This actually occurred on 10/5/05 but did not become a reportable injury until the employee had surgery on 5/15/06.

THE EMPLOYEE WAS GREASING THE SPARE RAYMOND MILL, THAT WAS BEING REBUILT. TO APPLY MORE PRESSURE ON THE GREASE GUN, HE PUT HIS LEFT FOOT ON A SPEED REDUCER TO PROP ON. WHEN THE PRESSURE WAS PL ACED ON THE REDUCER, IT FELL ON HIS RIGHT LEG. THE EMPLOYEE RECEIVED A CONTUSION, ABRASION OF HIS RIGHT LOWER LEG AND ANKLE.
