The operator was pushing material and constructing a ditch with a dozer in order to convey water away from the dig face. At the end of shift the operator reported to the pit lead that neck was sore when they tried turning head. Employee could not pinpoint a single event or time that caused the soreness.

Employee was enlarging a 9/16" hole in a stainless steel transition for a slide gate when the drill bit, bit on the edge of the hole and the drill twisted in the employees hand, twisting EE's wrist. Employee was fine but woke up the next morning with minor swelling.

Individual had a piece of metal fall and hit hardhat. It then cut the back of EE's neck. Six stitches. No lost time.

Left long finger was pinched between the Pellet Mill die and the compaction rollers which are located inside the die. The system was locked out and the employee was manually rotating the die while another employee adjusted the compaction rollers. Sutures were required.

The injured employee was trouble shooting the CB system. He disconnected a stainless steel line to check for flow. As the chemical began to flow, it struck another nearby line and a portion splashed into the employee's left eye. The employee's eye was flushed and then he was transported to Urgent Care. The employee took one dose of pain medication that night.

Employee packaging talc at four spout paper bag packer. Bag kicked off incorrectly, spout re-triggered resulting in stream of talc released outside of the bag. Employee entered talc stream to hit e-stop by spout . Got talc in his eye - flushed on site; further irritation so he was transported to local clinic where eye was flushed - given preventative antibiotic ointment.

WHILE CUTTING WITH A TORCH UNDER #1 DRYER, A HOT NUT LANDED ON EE'S LEG. THE HOT NUT BURNED THROUGH EE S COVERALLS, PANTS, AND BURNED LEG/KNEE. THE EE WAS WEARING KNEE PADS. THE HOT NUT BECOME CAUGHT BETWEEN KNEE PAD AND LEY/KNEE. THIS RESULTED IN THREE. 3RD DEGREE BURNS.

EE HAD BEEM WELDING GINDING AND DRILLING ON METAL AT DIFFERENT TIMES THROUGHOUT HIS SHIFT. EE DOES NOT KNOW WHEN FOREIGN MATERIAL METAL ENTER EYE. EE WENT HOME SLEPT AND WOKE UP WITH AN EYE IR RITATION. EE WENT TO CLINIC AND WAS TREATED WITHA METAL FLAKE IN EYE. METAL FLAKE WAS REMOVED AND EE RETURNED TO WORK WITHOUT ANY WORK RESTRICTION.
