Showing all 10Employee and contractor were installing a bucket elevator when it came free. Employee was tossed to the side and the contractor had the bucket elevator collapse upon contractor. Employee was released from the hospital after a few hours, and the contractor was sent to a hospital in Albuquerque.
While lifting with a crane, an elevator belt snapped and injured a contractor and an employee.
Employee was working on the Secondary Screen grinding on its rails. Employee advised employee felt something land in eye, but did not think anything of it. Employee advised employee was wearing safety glasses. Employee continued to work from the date of incident (Friday) into Tuesday 10/12/21 before advising what had occurred and that the eye was irritating employee.
Motor being lifted by chainfall, motor came loose & chain tightened pinching employee's fingers. Tearing skin on middle & pinky fingers, stitches needed on pinky finger.
While using cutting torch, slag burnt through pants and fell into left booting burning left foot

EMPLOYEE WAS CLEANING A PLUGGED CHUTE IN THE K SCREEN SPLITTER. THE EMPLOYEE REACHED INSIDE SPLITTER TO REMOVE ORE FROM PIPE. THE SPLITTER CAUGHT HIS COVERALL PULLING HIS HAND INSIDE AND CRUS HING HIS LEFT HAND.
EMPLOYEE WAS WELDING ON A TRACTOR IN THE SHOP WITHOUT GOOD VENTILATION AND THE SMOKE HURT HIS LUNGS.
EMPLOYEE WAS BAGGING 53 ORE FROM RAYMOND MILL WHEN PERLITE PARTICLE ENTERED RIGHT EYE. DID NOT SEEK MEDICAL ATTENTION UNTIL EVENING OF 4/25/00 AT E.R.
