EMPLOYEE ALLEGED HE WA PUMPING CP-158 DEGREASER FROM THE DRUM TO A 5 GAL BUCKET. THE BUCKET OVER-FILLED SPLASHING THE CHEMICAL ON FACE. DEGREADER GOT INTO HIS EYES. EMPLOYEE WAS NEGLIGENT I N WATCHING THE BUCKET FILL.

Baroid Drilling Fluids Inc · Caught in, under or between a moving and a stationary object

EMPLOYEE HAD ONE OF THE SIDE DOORS OPEN ON THE DRYER, REMOVING ROCKS, SCRAP, ETC. THAT HAD IT LOCKED DOWN. HIS LEFT HAND WAS RESTING ON THE DOOR FACING AND HE WAS USING A SHOVEL IN HIS RIGHT H AND TO FREE THE JAMMED PADDLES. THE PADDLES BROKE LOOSE ALLOWING IT TO TURN QUICKLY , PINNING HIS LEFT HAND BETWEEN THE PADDLE AND THE DOOR FACING, CAUSING LACERATION.
