Employee was gouging anchor in the pre-heater. The air coming out of the gouger hit a small pocket of material dust, which caused the dust to rise up under EE's welding hood and safety glasses. Employee was wearing all of the proper PPE for the task.

Employee was welding inside a steel chute and a hot piece of welding slag burnt through EE's pant leg and rolled down in EE's left boot and burned the top of EE's left foot in two places.

Employee was demoing an old bullnose setion inside preheater. As the piece was cut it fell, and one of the anchors caught his left knee and gouged/cut him. Employee thought that bullnose section still had one gusset attached when he torched the bullnose piece.

EE was operating a water lance w/8000 psi, when his left hand slipped off the handle in front of the wand. Cuts on fingers of left hand required 26 stitches to close.

EMPLOYEE WAS PULLING WATER LANCE BY THE WAND AND THE TRIGGER CAUGHT ON AN ACCESS DOOR AND ACTIVATED THE LANCE.. THE LANCE ABRAIDED THE LEG AND CAUSED A CUT WHICH NEEDED THREE STITCHES TO CLOSE . THE EE RETURNED TO FULL DUTY, AND DID NOT MISSWORK.

WAS REAMING OUT HOLES IN A STEEL PLATE, AND THE DRILL BIT CAUGHT IN THE HOLE AND SPUN THE PLATE. THE PLATE STRUCK THE TOP OF THE EMPLOYEE'S LEFT HAND CUTTING THE HAND. THE CUT REQUIRED 5 STITC HES TO CLOSE.

WHILE DRILLING A 1/4" HOE THROUGH A PIPE THE DRILL BIT BROKE DOWNWARD FORCE ON THE DRILL CAUSED THE RIGHT HAND TO COME DOWN ON THE DRILL BIT CUTTING THE THUMB ON THE RIGHT HAND. 3 STITCHES WERE REQUIRED TO CLOSE THE WOUND.
