Employee was removing a piece of angle from a wall. In the last cut the angle pinched the blade. When it kicked back, a piece of the wheel broke off lacerating EE's left bicep. Employee exited the job area and notified the tool crib attendant. Emergency procedures were initiated.

Employee was grinding a weld on the revolving frame & because of the shape of the workspace, sparks were moving around the room. Hood was down with the dark lense up. The clear lense would not seat and a piece of steel entered the employee's (R) eye & became embedded. Employee return to regular duty. EMPLOYEE DID NOT GO TO DOCTOR FOR TREATMENT UNTIL 8/19/06.
EMPLOYEE WAS HOLDING UMBRELLA TO KEEP WIND OFF OF WHERE MECHANIC WAS WELDING AND HE LOOKED AT THE WELDING FLAME.
EMPLOYEE WAS SHOVELING AROUND CRUSHER WHEN A ROCK CAME OUT AND BOUNCED OFF TIRE, HITTING HIM IN THE PELVIS.
EMPLOYEE WAS SHOVELING AROUND SCREEN PLANT AND SMALL ROCK FLEW OFF AND HIT HIM IN MIDDLE OF BACK.
EMPLOYEE WAS CUTTING DRAG CABLE WITH TORCH AND IT CAME BACK AND HIT HIM ACROSS LEFT ARM AND LEFT LEG.
EE WAS SHOVELING ROCK AWAY FROM POWER SCREEN & A ROCK FLEW UP & STRUCK HIM IN HIS UPPER BACK BETWEEN HIS SHOULDER BLADES
