Employee was using an electric stair cart to move a 55-gallon drum of oil. The cart tipped forward causing employee to strain their lower back. Employee was released back to work with temporary restrictions. No bending, lifting, carrying, pushing, pulling, kneeling, squatting, or twisting.

Showing all 27Contractor received an injury while power washing the inside of a railcar. While repositioning self the contractor slipped and accidentally squeezed the trigger on the power washer wand. The wand was aimed just behind the steel toe of EE's boot. The water jet cut through the boot and pierced the skin of the individual.
Employee was inspecting air slide and rotary feeder, when material became loose and blew out and underneath protective eye wear.
Supervisor was observing cleaning the CKG Bin. Supervisor stepped back to allow work to continue and was struck by an air blast from the exhaust of Jack Hammer. Air and dust particles went under Safety Glasses and were blown into the supervisors eye.
Tertiary air duct was being cleaned. Miner placed a jackhammer bit against equipment and stepped away to cool off. Miner returned to close the door to tertiary and bumped the jackhammer bit knocking it over, striking an air cannon. The air Cannon discharged and blew material out of tertiary, striking the miner. Miner was burnt on Chest, Neck, and Left Arm by flying debris.
Miner was using a handheld pipe threader to thread a 2" pipe, the threader jammed and caused the miner to be pushed vertically. The miner had arm locked in place and resulted in trauma to the Right Shoulder.
Miner was working with crew to clean up clink using a Vac Truck, while sucking up material the hose plugged causing it to collapse. Miner was struck by hose collapsing and swept EE off of EE's feet, causing EE to land on right arm/shoulder.
Employee stated was experiencing soreness in left wrist. Initial treatment was first aid, on May 20 doctor visit employee was given a wrist brace to use while at work.
Employee stated that as ee was installing roof bolts, ee felt a pain in elbow.
The employee placed small metal cylinder onto a vice grip and was using a handheld grinder to cut a notch into the cylinder. The grinder became wedged in the cut of the cylinder, jumped out, and cut the employee on the left hand. The employee bandaged the wound themselves and called the Safety Department. The employee has received six stitches.
Employee picked up high pressure water wand by the nozzle end, and trigger was pulled when it caught on a bracket. Water pump was off, but system was pressurized. Employee sustained HP water injection injury to right ring finger.
Employee was using jackhammer, and chunk hit the bit, pinching the employee's left index finger between the jackhammer and a door frame. Laceration required stitches.
Witness describes they (3) were lowering in a man basket when it suddenly and rapidly descended. Upon braking, EE hit his chin on the basket side rail, biting his lip. Crane operator immediately returned personnel to the surface and EE was taken off-site for medical treatment.
Employee was pulling air lance from a 4" diameter inspection port when he felt a burning sensation in the groin area. Employee was diagnosed with a hernia that required surgery to repair. Incident did not become reportable until 5/04/13 when he had surgery.
The Employee was operating a Roof Bolter, stretched reaching backwards and inadvertently stuck his fingers into the A/C vent, contacting the squirrel cage fan.
Miner was operating rock drill on 52' highwall, bit stuck & attempt to get bit unstuck,diagaonal section of the highwall sloughed,drill steel did not break,mtrl around drill steel pulled drill down with it approx. 25' & drill rolled over onto right side. Miner received laceration to forehead, laceration to right knee & contusions to right hip from wearing the seat-belt in drill.
Employee was welding on guard rail when his auto-darkening welding lens failed. He immediately replaced lens with a standard welding lens.
Employee was welding on finish mill shell (overhead and in awkward position). Felt pain in shoulder. (Did not become reportable until he started to miss work due to surgery on November 16, 2009.)
Side of dozer slipped when employee passed crest of stockpile causing him to be partially ejected from seat. Seat belt came unhooked (Seat belt was not defective, but was apparently not securely fastened.)
While moving mill grinding ball hopper into position, the ball on the hopper rolled over, pinching the employees hand between handle and ball. (Required stitches)
A brace was being hoisted by a crane. The connector was climbing into position. The flagman for the crane was not properly positioned and did not wait for the connector to get into position. The flagman brough the brace in too low and struck the connector in the back pinching him into a beam. He fell and his safety equipment caught him.

EE was replacing belt on air handler for waste fuels lab when he tried to stop pulleys by grabbing the belt resulting in finger being pinched between belt and pulley. unit was locked out and pulley was free-wheeling. stitches required.
EE WAS CUTTING LID ON DRUM WITH AIR CHISEL. USED HAND TO PULL UP ON LID AND METAL CUT THROUGH GLOVES.
INJURED WAS STARTING TO DRILL A PILOT HOLE, WHEN THE GLOVE ON HIS HAND BECAME ENTANGLED WITH THE DRILL CHUCK DRAWING HIS HAND AND FINGER ONTO THE DRILL BIT.
INJURED WAS JACKHAMMERING CASTABLE FROM THE WALLS OF THE CLINKER COOLER WHEN THE JACKHAMMER SLIPPED CATCHING HIS FINGER AGAINST THE WALL.
WHILE IN PROCESS OF POSITIONING BALL HOPPER UNDER MILL WITH TOW MOTOR AND CHAIN FALL, HOPPER SHIFTED ON CHAIN FALL CATCHING INJURED'S HAND BETWEEN FLOOR PLATE AND BALL MILL JOURNAL WALL.
