EMPLOYEE WAS OPERATING A NINE INCH GRINDER NEAR FACE LEVEL. HE WAS ATTEMPTING TO GRIND THE SURFACE OF THE BOOM. THE GRINDER CAUGHT AND KICKED BACK STRIKING THE EMPLOYEE ACROSS THE FACE & NOSE AREA. HE HAD DECIDED NOT TO USE THE REQUIRED FULL FACE SHIELD.

THE SCALER OPERATOR WAS INSTRUCTED TO BOOM DOWN TO CREAT MORE SLACK TO ALLOW THE CHAIN TO BE TIGHTENED AROUND THE BAR. THE SCALER HAS A FLOATATION VALVE THAT RELEASED BELOW A CERTAIN ELEVATION ALLOWS THE BOOM TO GO TO THE FLOOR. WHEN THE BOOM DESCENDED IT CAUGHT THE EE'S HAND.
