EE was attempting to straighten 12" U Bolt using a frame press with bottle jack attachment. EE turned U bolt on side to compress/reform bolt with a 4x4x2 piece of wood as a brace. When EE released bottle jack the stored energy caused the 4x4x2 piece wood to eject from the press, striking the EE in the face.EE was sent to emergency room for stitches.

Showing all 10Employee was using a Dewalt 6" grinder to cut/fabricate a piece of metal for support for a welding machine. Grinder kicked back and struck left hand. Injury was to the left hand middle finger, ring finger, and forefinger, just above the palm. Employee was sent to Lee Memorial Convenient Care where EE received 13 stitches to the fingers mentioned above. Restricted Duty prescribed.
Employee was grinding and welding at the 1260 Dragline. Employee was wearing face shield and safety glasses. Between the time he was welding and the time he returned to the shop had a pin size piece of debris enter eye. Employee washed out eye and thought all was removed.
Employee was operating a push mower and struck a rock in the grass. The rock came out the back of the mower, through the guard and made contact with his shin. He was sent to Urgent Care as a precaution and medical evaluation. He received three stitches to his shin.
Employee was working with a 4 1/2 grinding wheel adjusting safety chains on 4600 dragline. While cutting chain grinding wheel kicked back striking his left hand. Employee sustained a cut on the the knuckle side of his left hand, approximately 2 1/2 inch long. The injury required 3 staples. No loss of work, no light duty prescribed.
Clearing dense bush with chainsaw over head, chainsaw became tangled in dense weeds. Instead of turning it off, EE jerked chainsaw freeing it from debris causing the force to push the chainsaw downwards hitting his left knee.
EE was drilling fence post holes when the auger he was using hit a rock and jerked him forward pulling a muscle in his left leg.
The miner was gridding on a dragline bucket and piece of metal went in between his safety glasses and caused a cornea abrasion.
Miner was griding a seam in the dragline bucket and a piece of metal got in between this safety glasses by his nose and ledge in his right eye.
Employee was welding and cutting metal near our dragline without wearing his safety shield. He had been trained to do so in our weekly safety meeting. He received flash burns and the ER doctor gave him a Rx and to come back if he had pain. Employee was off on Sunday 12/2 but didn't report to work on Monday. He missed one day of work.
