Worker was positioning 10-inch plastic pipe into a 412 Fusion machine to be fused to another piece of pipe. On doing so, EE pinched EE's left pinky finger between the pipe causing a fracture to EE's knuckle.

Employee using a pedestal grinder in shop when EE's right thumb contacted the grinding wheel. Employee was treated at hospital and released.
Employee was welding on a metal strip which was caught in a feed chute of a crusher. The metal was cut free and sprung out hitting the employee and knocking the employee backwards.
A hydraulic hose on the employee's service truck outrigger got caught. The employee reached in to free the hose. EE began to retract the outrigger to gain slack in the hose and did not realize that a hand was in a pinch point. The outrigger broke several bones in the left hand.
Employee was in impact crusher welding - employee was on knees. When EE stood up EE felt pain in lower back and left leg.
Employee was under shaker holding the nuts for another Employee using an impact gun to tighten when pieces of dirt fell and went into the right eye causing it to irritate the eye
Putting lubricant in planetary of L270 loader using air operated pump. Pump developed air leak and stopped working. Employee was trying to find the air leak when piston came down pinching 2 fingers on EE's right hand.
The injured employee was struck by a horizontal support beam mounted on the metal decking as it was being removed at the primary crusher. The metal decking was initial struck and broke free shifting the weight.
Employee had greased drill rod threads, then reached in to remove dirt from table as Driller was moving drill rod carousel to drill rod catching employees right hand and forearm between drill rod and drill carousel causing bruising of forearm and laceration of right hand.
After cutting bushing off of shaft of speed reducer, he leaned over to look at the work, and while holding torch in left hand inadvertently moved torch over his lower right arm causing burn.
EE was using an air tool with a flapper wheel installed on it. He was working on a hoist cylinder for a haul truck. He was cleaning the packing seats to replace the packing. While cleaning the seat a particle of foreign material got in his left eye. He was wearing safety glasses, dust mask and ear muffs but admitted he should have worn a face shield or goggles.
EE was working on front suspensions of pickup truck. He was holding a breaker bar with socket on a nut while other EE was using a 1" drive air powered impact wrench to loosen nut when the breaker bar spun pinching ring finger between shock and breaker bar.
While operating D9H Cat Dozer in a rocky area and working boulders out; he was backing up, when the dozer took a hard bounce and his teeth went together, cracking one of them. Tooth # 19.
EE was helping co-worker with welding a plate for shaker 2 & something went into employee's left eye causing pain. He then went to Occupational Health & they referred him to eye doctor. He has a scratch on his left eye due to a foreign object. Must have flown into his eye. Eye protection was being worn at the time.
EE was washing off a concrete pad with a high pressure water hose, when hose broke and struck EE in the leg, causing injury.
EE WAS USING TORCH TO CUT A BUSHING INSIDE A BREAKER HAMMER WHEN IT BLEW BACK HOT GREASE THAT WAS TRAPPED INSIDE THE BUSING AND CASE CAUSING HOT GREASE TO LAND ON HIS NECK.
WHILE CUTTING STEEL PLATE WITH OXY-ACT TORCH, TIP SLAGGED OVER & THE TORCH WENT OUT. EE BRUSHED TIP WITH GLOVED HAND TO KNOCK DIRT AWAY & THE TORCH REIGNITED BEFORE HE COULD PULL HIS HAND AWAY .
EE WAS DIRECTING A CRANE OPERATOR WHERE TO PUT A LARGE PIECE OF CATWALK WHEN THE SLING BROKE AND LOAD DROPPING ON EE.
EE WAS USING A HAND GRINDER TO GRIND OUT KING PIN BORE WHEN FOREIGN OBJECT GOT BEHIND FACE SHIELD AND GLASSES. EE WENT TO DR AND DR DRILLED A PIECE OF STEEL OUT OF HIS EYE. (RIGHT EYE.)
