After utilizing a welder to weld plug holes in the feeder, eye began to bother them. They flushed their eyes and later that night was unable to open them so they sought medical attention and was given prescriptions.

Showing all 5Employee had performed maintenance activities that involved metal grinding. Left eye irritation resulted and flushing did not resolve irritation. Metal foreign encounter in eye had to be removed using drill and needle method. Eye doctor prescribed Prednisolone Acetate anti-inflammatory and Moxifloxacin antibiotic. Employee returned to work next day, full duty with no restrictions.
EE was removing the cutting edge bolds while using impact when a bolt slipped down and cut the employees fingers.
The Employee was performing maintenance tasks on a wash plant. EE went to kneel and EE's right knee contacted a grinder that was plugged-in and lying on the platform. The grinder came on causing it to tear through EE's coveralls and cause a laceration to the knee requiring stitches. The grinder was checked immediately and was in proper working order.
The employee was assisting another employee set up the fast pack. They lowered the jack leg while turning and employees right hand was crushed between the body of the jack leg and the jack leg mounting plate causing the injury.
