Showing all 14EE states he was tilting vacuum cleaner back to lie on its side to move under a conveyor belt. While doing so, his finger became caught between the vacuum cleaner frame and a handrail leading to an upstairs area.
EE states he was washing the KCl crude filter when he noticed the tank was filling too fast. He left the filter to shut off the water when the tank ruptured and sprayed hot brine on employee's ankle.
Employee was using radial arm saw to cut wooden 4"blocks. As he was pulling the saw to make cut, a block was ejected by the saw and struck his left thumb, causing a laceration. Incident being reported as Medical Treatment due to stitches.
Employee was cleaning end caps when his finger contacted a moving drive chain. This contact caused a tip amputation of the middle right finger.
EE received thermal burn from molten metal while welding on housing of a metal hopper. During the welding process, he changed his body position, which placed his right leg/foot in line of fire of welding process. The molten metal "popped" and particals went into his boot. Incident reported as restricted duty.
PRECISE ADJUSTMENT OF OILER REQUIRED POWER DUE TO SYSTEM DESIGN. WHILE MAKING THE FINAL ADJUSTMENT TO THE OILER LINKAGE ROD, THE EE WAS INATTENTIVE TO HAZARDS & PLACED HIS LEFT HAND ON THE OILER SUPPORT BRACE. HIS FINGER WAS IN THE LINE OF FIRE WHEN THE TUBE STRIPPER MOVED. HIS FINGER WAS CAUGHT BETWEEN THE OILER BRACE & THE TUBE STRIPPER.
EMPLOYEE STATES HE REACHED OVER THE STARWHEEL PLATE TO REMOVE A PEPPER CONTAINER FROM THE PEPPER SEAMER. WHILE DOING SO HE BECAME DISTRACTED, TURNED AWAY AND CAUGHT THUMB BETWEEN STARWHEEL FINGER AND CIRCULAR OPENING IN THE STARWHEEL BASE PLATE.
WHILE OPERATING THE CARTON MACHINE ON THE SECOND FLOOR, EMPLOYEE REACHED INTO THE MACHINE TO CLEAR A JAM. DURING THE PROCESS, HE PULLED HIS HAND AWAY FROM THE MACHINE AND CONTACTED THE BOX GUIDE RESULTING IN A LACERATION BEHIND HIS LEFT INDEX FINGER KNUCKLE.
Ee was assisting another ee in resecuring a vibrator screen. when tension was applied to the tension adjustment screws, the screen moved forward and struck ee s right thumb resulting in a linear fracture.
While tramming in scaler employee ran over chunk of salt causing him to lose his balance and fall to both knees.
WHILE MOVING THE HIGH-LIFT BASKET TO POSITION FELLOW WORKER DURING FACE TAMPING, EE DID NOT SEE OFFSET IN ROOF AND CAUGHT HIS OWN HEAD BETWEEN BASKET RAILING AND MINE ROOF.
WHILE USING A CHAINSAW TO CLEAR BRUSH ALONG RAILROAD TRACK, EMPLOYEE REPORTS HE WAS CUTTING A SMALL SAPLING WHEN HIS CHAINSAW KICKED BACK AND RESULTED IN A 1/2" LONG LACERATION TO RIGHT KNEE. THE LACERATION REQUIRED 1 SUTURE TO CLOSE.
A CAPSTAN WINCH WAS USED TO POSITION AND TIE-OFF BARGES UNDER A FIXED LOADING SPOUT. THE INJURES EE COULD NOT UNTIE THE MOORING ROPE FROM THE D3E-ENERGIZED CAPSTAN WHEN HE NEEDED TO REPOSITION A BARGE BECAUSE OF HIGH ROPE TENSION. THE EE USED AN UNAUTHORIZED PROCEDURE: HE STARTED THE CAPSTAN & KICKED ROPE WITH RIGHT FOOT. IT BECAME CAUGHT. ENTIRE LEG INJURED, AMPUTATED LOWER LEG.
