The production crew on shift was clearing a pre-heater tower plug in the feed pipe and stage 4. EE inserted Cardox tube into port and while trying to set the depth of the Cardox tube they felt pain in lower back on the left side. EE initially decided on self care however requested evaluation on 12/20/21 and was released to work with restrictions.

EMPLOYEE DID NOT VERIFY THAT ALL THE PRESSURE WAS EXHAUSTED FROM THE HYDRAULIC SYSTEM. WHEN THE EMPLOYEE ATTEMPTED TO REMOVE A HYDRAULIC HOSE THE COUPLING KICKED BACK AND CAUSED AN INJURY TO THE EMPLOYEE'S RIGHT HAND.
EMPLOYEE WAS GRINDING/WELDING ON THE LOADER BUCKET. HE NOTICED NOTHING OUT OF THE ORDINARY DURING WORK SHIFT BUT WHEN HE WENT HOME FELT A DISCOMFORT IN HIS LEFT EYE (FOREIGN BODY/FLASH BURN).
