Employee was operating a C5 568 roller compacting 2:1 slopes traveling up near the top of the slope when the engine shut down. The roller began to slide down the slope uncontrollable and gaining speed and when the roller reached the bottom the roller over turned on to its side.

Showing all 38Team Member was welding pipe on the pad & during the process the welding head shifted and smashed his right hand.
Employee was drilling a hole from the bottom up into a panel. His loose hand was too close to the drill bit and the slack in his glove got caught on the bit causing his hand to get pulled towards the bit. The employee's left middle finger was lacerated as a result.
The connector was hanging steel girders approx. 40' above the ground. The crane operator was swinging in a load girders. The worker had his back turned to the load, and due to poor communication between the worker, the other connector, and the crane operator, the load swung in and struck the back of his knee, which caused the front of it to hit the column in front of him.
Employee's were tripping out PVC when the under reamer slipped. Hitting the employee in that back of the arm. First report was employee hyperextended rt elbow.
We were washing the steel. We finished 1 section and boomed all the in. I then started turning the boom to wash the next column. Once I started turning I felt the lift starting to tip, we hit the column.
Washing the columns, the boom was extended so boomed all the way in. Once the boom was all the way in we started rotating the boom to get closer to the wall. At the moment of the rotation we were falling towards column.
Operator was backing up a ramp in his blade, bounced over a rock and jammed his lower back. We sent him to an authorized provider on 1/28/13. He tried to control the pain with ibuprofen for the past couple of weeks. He is receiving physical therapy a couple times a week.
Mechanic was using a crane to reinstall a track on a dozer, he got his finger smashed between the chain and the blade tilt cylinder, causing trauma to all 4 fingers on left hand.
DRILLER HELPER WAS STANDING ON THE FOOT CLAMP AND THEN THE DRILLER LOWERED THE HEAD WITH THE ROD SHOOT ON IT AND THE HEAD CAME DOWN ON THE EMPLOYEES FOOT.
A valve on a caustic pipe failed and sprayed the employee in the face with liquid caustic soda. A small burn to the right eye occurred and was treated by the authorized provider with antibiotic ointment.
EE did not report until 10/26/11. Shovel bucket struck the bed of truck and jarred Haul truck driver.
Grinder cutting wheel broke and struck EE in the chin. A face shield was being worn. The wound was sutured and EE returned to work.
Forearm contacted a spinning blade on a 4 1/2" grinder EE had just set down on a workbench.
Work glove was caught in a pedestal mounted grinding wheel. The first digit of ee's left thumb was injured.
EE was lifting a heavy bolt from the crusher spider with an overhead crane, when the lifting eye on the Bolt failed. The bolt pinched the first two fingers of his right hand.
Fingers struck by a rotating motorgrader fan blade
Tried to put wrench on drill pipe while pipe was still turning. Causing a sprain and abrasion to the little finger on the right hand.
Sabor sub came loose from the drill head. Injured put breakout wrench on sub to tighten it back up. The driller asked him if his fingers were clear & the EE replied yes. As hydraulic pressure was applied, the EE got his thumb between the breakout wrench & the drill mast causing an open fractur to the left thumb. Received stitches & placed on light duty until further notice.
IW was welding overhead when he felt a pain in his neck.
Driver was unloading lime from a pneumatic trailer. The product line on the trailer plugged with material. Employee attempted to use a 1 1/2 inch airline to blow or clear the product from the product line. The airline was placed in the product line. When pressurized the airline flew out and struck employee in the right knee.
Employee was making a drills l pipe connection when the drill pipe dropped and landed on his left wrist. Diagnosed with a contusion to the left wrist and placed on light duty for 5 days and the wrist was splinted for 3 -5 days
EE's finger was struck by an air conditioning fan blade.
Removing a bushing on the boom / stick knuckle of a Hitachi shovel when his toes were pinched between the stick and a crowd cylinder pin boss.
Employee was positioning drill pipe when the pipe slipped out of his hands, when the employee tried to catch the drill pipe his left hand was caught between the pipe and the pipe stand causing a nail avulsion to the little finger, a laceration to the little finger and fracture of distal phalange of ring and litte finger.
EE was removing a cutting edge bolt with and air wrench. The bolt spun and the bolt head cut two fingers.
Ee was tensioning belts on a sump pump when the pump started and pinched his left thumb between the belt and the sheave wheel. The tip of the thumb was partially amputated.
EMPLOYEE WAS USING AN AIR CHISEL TO CLEAN PACKED DIRT FROM AROUND A WEAR PAD IN PREPARATION FOR A WELDING JOB WHEN A FOREIGN BODY BECAME LODGED IN HIS LEFT EYE. THE EYE WAS IRRIGATED ON SITE. EE WAS SENT IN FOR MEDICAL CARE WHERE THE EYE WAS CLEANED. THE FB WAS REMOVED WITH A Q TIP AND NEOMYCIN OINTMENT WAS APPLIED.
EE CUT INTO 1/2" NAOH LINE & WAS SPRAYED W/CAUSTIC SOLUTION. STEPS HAD BEEN TAKEN TO DEPRESSURIZE LINE & PROPER SAFETY EQUIPMENT WAS BEING USED. WHEN CAUSTIC SOLUTION STARTED SPRAYING IT SPRAY ED UNDER FULL FACE SHIELD BEING WORN & ONTO EE'S FACE & EYES. FIRST AID WAS ADMINISTERED ON-SITE & MEDICAL TREATMENT PERFORMED. HIS EYES WERE IRRIGATED. NEOSPORIN OINTMENT WAS APPLIED TO LEFF
EE WAS ATTEMPTING TO CHANGE A DRILL BIT. HE HAD ATTACHED A CHAIN WRENCH TO THE BIT AND WAS HOLDING IT IN PLACE WITH HIS HAND AS HE REACHED INTO THE CAB TO OPERATE THE WRENCH HYDRAULIC LEVER. H E INADVERTENTLY PUSHED THE CAROUSEL INDEX LEVER INSTEAD CAUSING THE CAROUSEL TO INDEX AGAINST THE WRENCH CRUSHING HIS 4TH FINGER. THE FINGER WAS90% AMPUTATED & REATTACHED THE SAME EVENING. LT
EE COMPLAINED OF PAIN IN HIS THUMB AFTER MANUAL PULVERIZING. PHYSICAL THERAPY WAS FIRST PRESCRIBED ON MARCH 11, 2002.
EE WAS TIGHTENING A NUT WITH A TORQUE WRENCH WHEN THE SOCKET BROKE. AS HE CAUGHT HIMSELF TO KEEPFROM FALLING, HE TWISTED HIS KNEE. HE WAS DIAGNOSED W/ A STRAINED KNEE. PHYSICAL THERAPY WAS PRE SCRIBED & HE WAS RELEASED TO WORK.
EMPLOYEE JACKING DENT OUT OF 84" PIPE, HYDRAULIC JACK SLIPPED OUT CAUSING 6 BY 6 BLOCK TO CATCH LEFT HAND BETWEEN 6 BY 6 BLOCK AND PIPE WALL.
EE S FINGER TIP WAS PINCHED BETWEEN A DRILL BIT AND THE DRILL STEEL AS HE AND A HELPER WERE ATTEMPTING TO CHANGE A DRILL BIT. HE WAS EXAMINED IN A LOCAL EMERGENCY ROOM WHERE THE FINGER WAS X R AYED, CLEANED SUTURES, BANDAGED AND SPLINTED. THE X RAYS SHOWED A FRACTURE OF THE FINGER TIP. HE WAS RECHECKED THE FOLLOWING DAY AND RELEASED TO FULL DUTY.
WHILE ADDING A DRILL CASING, THE END OF THE CASING SLIPPED AND FELL TO THE DRILL SLIDE, INJURING EE'S LITTLE FINGER. LITTLE FINGER ON RIGHT HAND SMASHED, TIP OF BONE CRACKED, TENDON CUT. LITTL E FINGER IN SPLINT FOR 7 DAYS.
A DRILLER WAS ATTEMPTING TO REMOVE THE DRILL HAMMER WITH A HYDRAULIC CHAIN WRENCH. WHEN PRESSUREWAS APPLIED TO LOOSEN THE HAMMER, THE NUT FLEW OFF AND STRUCK THE DRILLER ON THE EAR & MASTOID BONE. HIS EAR WAS LACERATED AND BRUISED ON THE MASTOID. THE LACERATION WAS SUTURED WITH THREE SUTURES.
EE WAS MAKING A ROD CONNECTIONS. HE WAS WAERING LOOSE CLOTHING WHICH WRAPPED AROUND THE PIPE AND SPUN HIM AROUND. AS HE SPUN AROUND, HE HIT THE DRILLER, IN THE SIDE OF THE KNEE, CAUSING HIS KN EE TO BEND INWARD.
EE WAS MAKING A ROD CONNECTION. HE WAS WEARING LOOSE CLOTHING WHICH WRAPPED AROUND THE DRILL PIPE & SPUN HIM AROUND. AS HE SPUN AROUND, HE HIT OTHER SESSIONS, THE DRILLER, IN THE SIDE OF THE, CAUSING OTHER'S KNEE TO BEND INWARD.
