While performing bucket repairs on electric shovel 343, welder was grinding in a corner of the inside of the bucket. The grinder kicked back causing the employee to lose his balance and contact his left hand with the grinding wheel. Although the employee was wearing welding gloves, the wheel cut through the glove and into the middle finger knuckle, requiring two stitches.

Showing all 14Employee reported that he had received flash burns, from reflection, while welding on his previous shift. Employee was evaluated at the hospital, given prescription eye drops to treat his eyes, and released to return to work.
Employee used plasma cutter to notch out some tubing. Employee did not use adequate lens for task, resulting in flash burn to his eyes. Employee did not realize he had received flash burn until later that evening. Employee missed one shift (10/7/12) as a result.
