Two employees were welding wear parts onto an excavator bucket. The injured employee received flash burn to his eyes from the welding arc reflecting into the back of his welding hood. The employees had failed to recognize the hazard and did not place any kind of screen for protection.

Employee was grinding on a horn in an impactor when a piece of debris got in his eye under his safety glasses.

EE WAS A MEMBER OF MAINTENANCE CREW, PERFORMING PREVENTIVE MAINTENANCE ON A PRODUCTION HAUL TRUCK. THE EE'S JOB WAS TO REMOVE MUD FROM THE UNDERSIDE OF TRUCK, WITH HIGH PRESSURE WATER HOSE. EE WAS CARRYING OUT HIS NORMAL DUTIES; WHEN HE EXITED, FROM UNDER TRUCK, HE NOTICED HIS LOWER BACK WAS IN PAIN. HE HAD TWISTED HIS LOWER BACK.

EE WAS LOADING DRILL HOLES, HE POSITIONED THE AUGER ON THE DRILL TRUCK OVER HOLE. IT SWUNG BACK STRIKING EE RIGHT ABOVE THE LEFT KNEE. THE JOYSTICK TO THE AUGER CONTROL MECHANICALLY FAILED CAUSING THE ACCIDENT.
