Was using water blaster cleaning cement riser. Chunk fell from side of riser hitting the tip of the water blaster nozzle.
Cleaning K2 tower 3rd floor, Northwest corner, top door. Cleaning with 4' water lance shot into and hot dust puffed back hitting employee's right side of jacket. Heated jacket and caused burn to arm.
EE was packing on B-line and #4 spout bag blewup, causing cement to go into left eye.
The victim (contractor) and 2 Cemex employees were trying to clear a plug from the K2 preheat tower. As the plug broke free hot material discharged out the Cardox port and impinged on the victim causing burns.
Employee was cutting bolts overhead with a torch. Slag fell on to the employee. The employee stopped cutting and removed his goggles to clean the built up slag off of them and a piece of slag fell in to his right eye. Employee required prescription eye drops.
Employee was hooking up water blaster gun next to riser on preheat tower when an air cannon discharged blowing air and material through an open port causing a mild burn on the face and getting material in the eye which had to be removed at the medical clinic.
EE was cleaning walkways near/around JBC8. While using a jackhammer EE smashed his hand in between Jackhammer and metal shoot. Causing a dislocation of pinky.
EE was trying to fix a stuck pallet in the pallatizer when he got his arm stuck between the rollers and the pallet came loose and arm was caught. The result was crushing of his forearm and wrist resulting in damage to muscles and possible ligaments.
EE was cleaning north side of tower with an airlance when hot feed blew onto his jacket on right side burning a hole through to his right forearm and wrist causing 2nd degree burns.
EE was using a grinder on a flange on Silo 14 Blade. When the blade was pinched causing the grinder to come out of his grip it swung around and cut the top of the EE's right hand.
EE was using the jackhammer while working under JBH3F as he repostioned the jackhammer he made a quick turn and hit his tooth on some conduit that was hanging down from the baghouse. This chipped his right front tooth.
EE was cleaning Kiln #3 feed shelf with a waterblaster when from the port of tower a puff of hot kiln dust came out. The dust went under face shield and burned his chin and lips.
While EE packing cement on B-line a bag broke and cement dust got into eyes causing irration and a scratch on cornea of right eye.
EE was packing bags of cement on A-line when the top of the bag of cement broke shooting dust into the air and EE got dust into his eyes. EE washed eye out however left eye was irritated. EE received a prescription eye drops.
EE was closing elevator door on K2 (kiln) elevator. While door was closing EE attempted to push strap to the outside of door and door slammmed on left hand.
EE was airarcing inside of GCC2 and got a foreign object in left eye.
Internal audit found need to report. EE was working on the packing line when the palletizer stopped. EE want into the palletizer with out locking out the machinery and while fixing the problem his leg got stuck between the layer pusher and the layer press.
EMPLOYEE WAS PERFORMING DESCALING OPERATION ON PREHEATER TOWER. A BLOWBACK SITUATION OCCURRED RESULTING IN FIRST AND SECOND DEGREE BURNS TO NECK AND CHEST.
EE WAS TYING A PORTABLE FAN AT THE SECOND STAGE PREHEAT VESSEL. THE ROPE GOT ENTANGLED IN THE FAN AND WRAPPED AROUND THE END OF HIS LEFT RING FINGER. THE LITTLE FINGER ON HIS LEFT HAND WAS ALS O INJURED. THE RESULT WAS HE LOST THE TIP OF HIS RING FINGER.
EE WAS CUTTING BLOCK WALL WITH GAS POWERED DIAMOND WHEEL CUT-OFF SAW. EE WAS ON SECOND RUNG OF LADDER. HAVING FINISHED CUT, HE KEPT ONE HAND ON SAW, PUT OTHER HAND ON THE LADDER TO CLIMB DOWN. HE BUMPED THE LADDER WITH THE SAW & WHILE THE BLADE WAS STILL SPINNING THE BLADE HIT HIS LEFT KNEE CUTTING IT. THE CUT REQUIRED 21 STITCHES.
WHILE WELDING FROM SCAFFOLDING ABOVE A WEDGE BROKE AND FELL. STRUCK HIM ON THE RT. SHOULDER.
REAMING OUT BOLT HOLES WITH IMPACT GUN, SOMETHING FLE INTO HIS RIGHT EYE.
