Employee was setting up the welding leads and was checking to make sure ground was hooked properly. He struck an arc at when injured employee walked up with his hood up and received welders flash.
Employee was taking care of a water pump at 3-3 cooling tower basin. Employee noticed a small oil leak and started to look for it and at the same time clean up the oil. He was using several rags and he placed his hand too close to the radiator fan. The radiator fan then sucked up the rags and his hand into the fan causing a laceration between two of his fingers on his right hand.
Employee was operating a nut runner when it slipped out of his hands and hit employee in the left shin. EE reported incident on 12/14/14--incident happened on 12/10/14 no report made on 12/10/14.
EE was assisting fellow EE with closing the discharge lukie on 5-8 heater using a nut runner. As the valve was almost closed the nut runner torqued up, the handle swung around and hit EE on the right wrist and right collar bone. AC Joint Separation.
Employee was about to hose down on the pad at 5 Hytank but when he tried his hose plugged off. He worked the valve to see if it would pass through and it did. He lost control of the nozzle when it became pressurized again blowing off his hard hat safety goggles, and glasses getting DFI in his eyes and nose. Conjunctiva and facial irritation.
Employee was rolling surry blinds using an Impact wrench 2950 approx weight 50# with socket. While shooting top nut that was slightly worn impact slipped off nut he attempted to keep control of impact so that not to drop it causing strain to right shoulder.
Employee was closing the gate valves on 2-2 ESP dust screw using a pneumatic air gun, when the air gun caught his left glove causing his wrist to over torque. The glove tore away.
Opening feed Lukie on 3-8 flash tank. EE was opening the feed Lukie on 3-8 flash tank using a "nut runner" and two multipliers. EE states that when he actuated the nut runner the scaffold he was standing on shifted slightly. The shift caused the nut runner to move where he had it braced with his leg and allowed it to pull on his right shoulder. Shoulder Strain
Employee was using a cordless saw to cut a fitting and caught left little finger in the saw. Sutures required.
Operating Long reach excavator in South "B" Bauxite Building. Excavator turned over onto its side. ERT was paged out to help EE exit the cab of the excavator. Concusion
Employee was using hydro blasting spin nozzle and the nozzle got diverted and shot employee in left shin with a burst of water lacerating the skin.
While drilling out 3-17 outboard heater drain valve, the drill bit broke and drill and broken bit struck EE in left palm.
Employee has been de-scaling a line with a rivet buster. He set the rivet buster on a pipe in order to take a break. It slipped off of the pipe and landed on the employee's left hand, fracturing his ring finger.
A T.S.S. employee was in the process of shutting down a pump, while the number 1 washer feed line was being hydro blasted. As the employee was performing his task a piece of scale came loose from the area being hydro blasted and struck the employee on the chin causing a small laceration.
EE was sitting on a bucket and welding overhead. Several pieces of welding slag fell and landed in his groin area. EE sustained 2nd & 3rd degree burns in his groin area. EE placed on restricted duty.
EE was rolling slurry blind on #5-9 heater, when the impact gun that he was using slipped off the nut. The gun pinched employee's right hand ring finger between the gun and slurry blind. EE received a deep bruise to the tip of his finger. X-ray revealed fracture on finger tip. EE placed on restricted duty on 10/08/12
EE was using a 3/4" air impact with a "wobble" joint on a 1 13/16" socket. EE placed his left hand around the wobble and socket assembly to stabilize it. EE's left index finger at the tip got pinched by the wobble joint. EE was treated at medical and placed on restricted duty. EE RTW full duty on 9/17/12.
While using a rivet gun to conduct clean up under #5 Digester Unit. The 18" rivet gun bit broke, fell forward and struck the EE on the top of his right foot. The force caused the bit to cut through the boot & punctured the EE's foot. EE was taken to onsite medical facility where he was treated & released. EE was placed on restricted duty on 10/10/12.
While a fellow EE was begin to use a die grinder nearby, the grinding disc broke apart and a piece struck EE in the upper chest area. EE was sent to the onsite medical facility for evaluation. EE placed on restricted duty on 8/24/12.EE RTW Full Duty on 9/6/12.
EE was using an air lance to clean off the west apron feeder on the north hopper. When the EE pulled on the air hose, the hose hooked onto the apron feeder guard and caused it to fall on his ankle. EE was sent to an offsite physician for examination and treatment. EE placed on restricted duty on 06/29/12.
While working to remove a 2" threaded connection EE felt pain in upper back/neck area. EE was using a 3/4" impact in tight, confined space. EE was sent to the mine medical facility for evaluation. EE sent to outside physician on date of incident. EE placed on restricted duty on 6/1/12.
While operating a bulldozer, the bulldozer stalled and came to a sudden stop (bulldozer had been traveling down an incline while breaking over on a stockpile peak). EE felt the back of his neck "pop". EE placed on restricted duty on 03/02/2012.
As employee was trying to close a filter press he states that the air motor broke off the mount. When the motor fell off it struck the air valve causing it to open and energizing the bull gear. The ratchet swung around striking the employee on the left hand. X-ray revealed fracture of two fingers in his left hand.
EE was resetting the a motor on the south E-crane. While he was doing this another EE was operating the E-crane and filling the hoppers with bauxite. When EE discharge bauxite load, the hopper shook and some rocks slid off striking EE in the right hand and the upper back area. EE placed on restricted duty on 12/28/2011.

EE was using a drill to drill out a bolt. The drill bit broke and the EE's right shoulder thrust forward. The sudden action caused a sprain/strain on the right shoulder. EE was placed on restricted duty on 04/28/11 by his treating physician.
EE was working to clear a pluggage on a bauxite bin when his left middle finger was caught between a water lance and the bin structure. EE was taken to local hospital where Xray was negative for a fracture. On January 17, 2011, Orthopedic Surgeon diagnosed fracture of the left middle finger, making this a Recordable Injury charged to 2010.
EE was using a pneumatic tool to remove scale from the floor of a tank. A piece of scale got into EE's left eye, causing minor irritation. EE was wearing safety glasses with side shields and a full face shield. As a precautionary measure, EE was given Rx medication for the prevention of infection. No lost time or restricted duty.
EE isolated the air source to an air powered hand tool. EE started to de-pressurize the air hose and tool by depressing the air tool trigger. Air tool socket spun off, striking EE on his right lower leg, causing a deep contusion and a small laceration. On 08/23/2010, treating physician took EE off work due to infection.
Employee was using a scaling gun at 30-7 sump. The bit got stuck in scale. As he attempted to pull the bit out of the scale, he felt a pop in his lower back. Lower back strain; placed on restricted duty.
EMPLOYEE WAS USING A WATER LANCE TO UNPLUG 2-1 PANT LEG ON #2 BAUXITE BIN. EMPLOYEE STATED THAT A PIECE OF SCALE BROKE, AND STRUCK THE TIP OF THE WATER LANCE, CAUSING HIS RIGHT ARM AND SHOULDER TO JERK IN A UPWARD MOTION. EMPLOYEE FELT A SHARP PAIN TO HIS RIGHT SHOULDER. EE Treated & released at plant medical facility. On followup visit of 11/2/09, EE ws placed on restricted duty
EE was milling a valve part on a Lucus lathe when he felt an irritation in his right eye. EE was treated and released. On 7/28/09, EE had irritation on his right eye. EE was given an Rx for an antibiotic against infection, making this a recordable injury. No lost work time.
Employee was drilling the top level tap on #3-3 flash tank when the drill bit got stuck. The drill then back spun (counterclockwise) and it was at this time that employee felt a pain to his right shoulder. EE taken off work on 08/20/09.
Employee was removing bolts from a flange. He was operating an air operated impact tool with right hand while holding back-up wrench with left hand. The socket bounced off the nut and twisted his forearm.
Using impact gun to remove flange bolts and the impact gun slipped off the nut and resulted in shoulder strain.
On 10/16/08, employee was using an impact gun with his right hand in the trigger when he began experiencing pain to right wrist. Employee received prescription medication on 11/18/2008.
Employee was using a water lance to unplug a feed chute to 3-1 weigh belt. Employee was holding the lance pointing it upward when a piece of scale fell striking the lance. Employee states this caused the lance to jerk up while he was still holding it and caused a strain to both shoulders.(shoulder surgery refer due to day off by 220090630041)
Using an air impact gun to loosen flange bolts.
Employee pinched thumb between a rivet gun tool bit and a heat exchanger head while pushing bouls out that had become bound from process scale.
EE WAS WORKING AT TOPSIDE CHARGE HEADER, WELDING PIPE. SLAG FELL INTO WELDING SLEEVE OF [R] ARM CAUSING 2ND DEGREE BURN TO [R] FOREARM. NO LOST TIME OR RESTRICTED DUTY.
EXCHANGE USED AN IMPACT GUN [APPROX. WT. 60-70#] FOR ABOUT 35 MIN. CAUSING VIBRATION AND FELT A SHARP PAIN TO LOWER LEFT BACK AND BOTH LEGS GAVE OUT CAUSING HIM TO STUMBLE BUT NOT FALL. EE C/O PAIN TO LOWER LEFT BACK RADIATING DOWN LEFT LEG, EE TAKEN OFF WORK BY HIS DOCTOR.
EE WAS CLOSING AN ANGLE VALVE ON #2 DIGESTER FEED TANK. WHILE USING NUT RUNNER THE GUN SPUN AROUND AND HIT EE ON HIS HARD HAT CAUSING A LACERATION TO LEFT SIDE OF HIS HEAD. LACERATION REQUIRED FIVE SUTURES TO REPAIR. NO LOST WORK DAYS OR RESTRICTED DUTY.
EE was using a scaling gun to descale a bauxite discharge chute. The gun got stuck in the scale & when EE pulled on the gun to get it loose, he felt a pain in his lower back. EE placed on restricted duty on 5/29/2007.
Employee was hydroblasting using a standard length shotgun in the 22-1 sump pump in the Rodmill area. The employee cut the top of his left foot with the water jet from the shotgun resulting in a 1 1/2 inch long cut approximately 1 1/2 inches deep. This injury resulted in a workers compensation claim and loss time.The length of lost time has yet to be determined.
Employee was on an overhead grated walkway walking toward a stairway. A tank below vented steam immediately to the side of him. His lower left arm sustained a second degree thermal burn. No Lost Work Days or Restricted Duty.
EE was standing adjacent to a 1 inch steam trap discharge pipe. The pipe discharge was a ground level. Hot condensate was discharged from the steam trap and contacted the back of the EE's left foot. Result was a second degree burn to the back of the EE's left foot. EE was placed on restricted duty on 5/8/06.
EE was using an air chipper to remove scale from inside a digester tank. A piece of scale fell onto his right arm causing a cut on his forearm. The scale was about 12" x 18" x 2" thick.
EE was attempting to install a safety clip on a pressured one inch air hose. The hose became uncoupled and the EE's face was sprayed with air and residual caustic that was trapped in the hose. EE was treated at a local hospital. Treating physician issued prescription medication.
EE was removing a set of screws from a pump coupling. Pump was down, drained and de-energized. EE's left gloved hand got caught on one of the screws and his hand got twisted. This resulted in a fracture on the left hand finger tip. EE placed on restricted duty.
