Employee was using 12 volt tester. The tester slipped off temp switch and came in contact with fan blade. The fan blade struck tester and drove blunt end into palm of right hand requiring 16 stitches.

EMPLOYEES BACK WAS HURTING WHEN HE GOT UP THE NEXT MORNING AFTER WORKING NIGHT BEFORE. EMPLOYEE DID NOTHING OUT OF ORDINARY ON HIS NORMAL SHIFT.
