Equipment was locked out while employees were flushing ice from sand screw. Later employee rolled the vee belts backward to unplug east screw. Sand screw motor was energized. Vee belts pulled employee's hand into belt guard, cutting EE's hand & breaking 2 fingers. Employees had removed their locks to bump the equipment when this occurred.

WORKER STATED HE WAS USING METAL LATHE TO CLEAR THE BIT OF STEEL WITH LATHE LOCKED OUT. THE PIECE OF METAL HE WAS REMOVING CAUGHT ON THE BIT AS HIS HAND WAS PULLING ON IT AND HE CUT HIS THUMB.
EE WAS WORKING AROUND CO-WORKER WHO WAS WELDING AND GRINDING. A PARTICLE GOT INTO HIS EYE.
