FOLLOWING A REPAIR TO THE ROLL SCREEN, THE INJURED TURNED THE MACINE BACK ON TO TEST THE OPRATION. THEN EE ATTEMPTED TO REINSTALL THE GUARD WHILE THE MACHINE WAS RUNNING. EE'S GLOVE WAS CAUGHT BY A SPROCKET, PULLING EE'S FINGERS INTO THE SPROCKET AND CHAIN.

Showing all 3Miner was using a portable hydraulic ram to remove a bushing. A sudden movement of the ram caused the victim's arm to be twisted backwards. Miner worked remainder of the shift & rested over the weekend. Upon return to work Monday, the miner was hit in the shoulder/arm with an entrance door at the processing plant. The 2nd incident caused the miner to seek medical attention.
EE WAS WELDING AND WAS WEARING PROTECTIVE GEAR, BUT APPARENTLY HE GOT FLASH BURNS FROM OTHER WELDERS IN THE AREA.
