INSTALLING PIN IN LEFT WING JACK 13 MINER. EMPLOYEE HAD A HEADER BOARD UNDER JACK TO ASSIST IN ALIGING JACK AND HAD A BOLT WRENCH IN FRAME OF SHOVEL AND THROUGH EYE OF JACK. HE HAD COWORKERS MOVE JACK HYDRAULICALLY TO ALIGN HOLE UP. IN THEPROCESS HIS FINGERS GOT CAUGHT IN THE JACK CAUSING LACERATION AND A FRACTURE.
