Employee was drilling 1-5/8" holes and when in the process of changing out the drill bits, the head released and pinned EE's Right-Hand little fingertip between the Jacobs chuck and the plate EE was drilling. This caused a partial amputation of the Right-Hand little fingertip.

Hydraulic ram was placed on a lug mounted to the side of the dragline. The ram slid off the lug. The safety line attached to the ram caused it to swing and contact EE's left hand.

While brush cutting with tractor and 15 foot brush cutter operator hit an unseen rock which was propelled from the cutter through the back glass of tractor striking the operator on the right side of head causing a laceration. Operator was transported to clinic where laceration was stapled shut.

A contractor employee dropped a 1" impact approximately 2' to 3' and struck a Luminant employee on the lower left side of EE's back, just under the bottom rib. Physician performed x-ray with negative results. Employee was released to work with restrictions.

Employee was air gouging to remove the wear plate from a D9 dozer's blade. As employee was making the last cut, the stored energy released the top of the plate and it struck the employee on the right shoulder causing them to lose balance and fall/ Hard hat came off and head struck the new wear plate which was staged nearby. Employee has 5 years mining experience.

EE was drilling holes in a dragline swing case seal so it could be removed. In the process,felt something in left eye and tried to wash it out. EE was transported to the ER. Next day EE was seen by a eye specialist to remove a small piece of metal from the eye, a prescription was given for pain/eye drops and EE was instructed not to return to work for at least 24 hrs.

Fans were turned on inside the D-35 dragline machinery house after a 4 day outage. The fans caused dirt/debris to enter the right eye of a mechanic who was working near the #1 swing. EE did have safety glasses on. EE didn't report the event until 11/6/2017 as eye continued to worsen. EE received prescription eye drops from the injury.

Mechanic was trying to cut a 3" rod bolt off that was stuck. Was using a portable saw and had made two cuts. The saw blade caught and kicked the saw back cutting thru the glove cut left ring finger at the first joint. 3 stitches were applied. No restrictions. All PPE was properly being used-Face Shield, Gloves and hearing protection. Saw had all safety devices in place.

Carrier retainer bolts on 8750 dragline were being removed with a 1" air impact and a 2" socket. 2 or 3 bolts had been removed and on the next bolt the socket broke into 2 pieces. Part of the socket contacted left wrist, EE didn't release the impact and it torqued EE's right elbow. Restrictions were issued by Dr. and this became recordable and lost time.

Injury occurred when two employees attempted to pull a F-150 truck out of the muddy pit. EE was removing the tow strap from the truck when the dozer went into motion. EE was knocked down by the truck and then drug about 75'. EE was then taken to the hospital for care.

Employee was working on a dragline hoist drum using a 1 1/2" impact wrench to remove bolts when he felt pain in his right shoulder. On January 12, 2015 he was diagnosed with tendonitis in his right shoulder.

The injured was cutting 6" dragline cable with a cutting torch into sections to be loaded in a truck. When the cut was completed the cable sprung apart at the cut, slapping him in his right leg injuring his knee.

Employee was cleaning coal in the pit on a D10R dozer. At approximately 12:30pm he started experiencing pain in his lower back. He did not report his condition to his supervisor as an on the job injury until April 11, 2014. The injury became recordable on April 17, 2014 when he was issued work restriction by a doctor.

An employee was attempting to remove a wellhead from it's position by cutting through the casing with a cutting torch. The employee cut several little openings in the casing and the wellhead fell over and struck the employee due to a lack of support of the wellhead by a backhoe that was being used to complete the job.

On 1-29-13 employee was operating motor grader when blade hit a rock causing the operator to jerk her back. On 2-26-13 employee begins losing work days causing injury to be reportable.

Employee was operating a CAT D-9T Dozer on a coal stockpile for the shift. At the end of the shift, the employee reported stiffness to the lower back after 12 hours of pushing coal. The employee asked to see a Chiropractor and was prescribed 3 visits of adjustment thus making this injury reportable.

Employee was gouging clad plate on the morning of August 3, 2011 at the 3 Oaks Mine. While gouging the plate a piece of the gouging rod end broke off and landed on the cuff of his jeans. By the time he had raised his welding hood to find the "Red Hot" carbon rod, the rod had burnt through his pants leg and made its way into his boot.

The mechanic was adjusting the drag brakes in preparation for lowering the dragline boom. The employee was using a one inch drive impact wrench with a 2.25 inch socket to compress the brakes spring. As he was tightening the spring nut, the socket broke apart causing a laceration to the employee's upper right arm. Six sutures were required to close the wound.

Winfield North - D-21 Demolition. EE was cutting sheave from A-Leg cross piece. In wrong place and sheave dropped on lower rt leg & foot. 4 stitches & 12 staples. No broken bones.

Cat 777D end dump was being loaded by an EX1900 Hitachi. Hitachi operator was blinded by the sun momentarily and the bucket of the backhoe hit the back of the end dump jarring the end dump operators neck. He was diagnosed with muscle spasms and a muscle strain of the neck and received two days of at home rest starting 3/1/11. This met the criteria for an MSHA recordable.

A mechanic was installing tires on haul truck. As he was tightening the clamp up on the wheel the tire turned a bit and his finger was underneath the clamp. He smashed his finger when he ran the inpact to tighten. The tip of his right index finger was fractured and he also required 3 stitches to close the laceration.

Employee was welding under the walking cam while another employee was machining on the bore of the walking cam. Employee felt something hitting him on the back and reached around to see what it was and lacerated his right index finger with a shaving.

Employee was lancing and a hot slag fell into the glove burning the back of right hand. At the time of incident the injury was considered a First Aid. On 07/08/2009 the employee was treated for an infection resulting from the injury which made it reportable.

Employee was removing an old gasket with a buffing wheel. Safety glasses and safety goggles were being worn by the employee. The employee had also installed a fender on a scraper. He was wearing safety glasses while doing this task. The employee received some foreign matter in his right eye while conducting one of these task.

Employee was in process of sawing through a PVC pipe that was located in an area that was causing tension on the pipe. When the employee cut completely through the pipe, the tension caused the pipe to snap up and trap his finger between it and a metal support. This caused a laceration on the top side of the right index finger of the employee.

When removing existing power poles, sandy soil along with poor placement of crane caused rear outrigger to giveway when pole was removed. When the outrigger pushed into the pole hole the crane became unlevel & overturned. The operator was attempting to get clear of the overturning crane when he fell from the crane hitting ground causing sprains to neck/back/shoulder

Employee was operating a D10 dozer. He was ripping out several rocks and when backing up, the dozer slipped into a hole. This caused the dozer to jar, resulting in an injury to the employee's back.

EE could not reach coal with lancer, EE went in the coal bin throught the side hatch door, EE worked in postion for about 30mins standing on the coal pile working the coal down the shute with the lance when suddenly the coal dropped quickly sucking him down into the coal fines up to his neck.

While lifting a 300 lb. shaft with the shop overhead crane the rope used to rig the shaft to the hook of the crane broke and the shaft fell striking the employee on top of his right foot, fracturing the little toe and the one next to it. No restrictions were given and the employee returned to work.

Thr foreman walked into Rev Fr to check on crack being welded. Welder stepped from 3' ladder causing air gouger to fall to floor hitting trigger...sending debris from the floor into the air and into the foreman's left eye.
