SA BLADE BACKED3OVER THE P & H SHOVEL BALONEY CAUSING A POWER FAILURE. THE SHOVEL WAS LOADING AN R-100 TRUCK WHEN THE POWER FAILURE OCCURRED. SWING BRAKES OPERATED CORRECTLY BUT INERTIA CARRIE D THE LOADED BU7KET INTO THE RIGHT REAR CORNER OF T HE TRUCK BOX JARRING THE OPERATOR.

I,NAME, MECHANIC-WELDER, WAS ATTEMPTING TO REPLACE A HOIST CABLE ONTO A HOIST SHEVE. HE THOUGH THE CABLE WAS SLACK AT THE TIME OF REPAIR. WHILE POSITIONING THE HOIST CABLE IT BECAME TIGHT AND PINCHED HIS HAND BETWEEN TWO CABLES. THE INJURY REQUIRED SUTURES TO BE PLACED. THE INJURED MINER DID NOT FOLLOW ESTABLISHED SAFE WORK POLICIES.
