THE EE WAS WORKING ON A PUMP THAT SUPPLIES CAUSTIC SODA TO A WATER SYSTEM FOR THE PREP PLANT. HE LOOSENED A HOSE THAT HAD PRESSURE IN IT & CAUSTIC SODA BLEW INTO HIS FACE CAUSING CHEMICAL BURN S TO BOTH EYES FACE & NECK. THE EXTENT OF DAMAGE TO T H E EYES IS NOT KNOWN AT THIS TIME.

OPERATOR WAS CLEANING DRILL TABLE WHIEL DRIL WASRUNNING THE PULL DOWN CHAIN CAUGHT HIS HAND PULLING IT INTO THE SPROCKET THE FIRSTR AND SECOND FINGERS OF LEFT HAND WERE AMUTATED AND THE THIRD AND FOURTH FINGERS WERE CRUSHED

EMPLOYEE WAS WELDING WITH A PORTABLE WELDER THE GOVERNOR STUCK CLOSED AND HE ATTEMPTED TO OPEN THE GOVERNOR BY PUSHING ON THE GOVERNOR BY PUSHING ON THE GOVERNOR ARM HIS HAND SLIPPED & HIT THE FAN BLADES CAUSING CUTS TO 3 FINGERS OF THE RIGHT HAND
