WHILE LOADING A TRUCK WITH LIGNITE WITH A SHOVEL, HOOTEN ENGAGED THE HOIST LEVER WITH HIS HAND WHILE SIMULTANEOUSLY RELEASING THE HOIST BRAKE PEDAL WITH HIS RIGHT FOOT. THE PEDAL THEN JUMPED U PWARD STRIKING THE BOTTOM OF HIS RT. FOOT. X-RAYS SHOWED HIS RT. ANKLE TO BE FRACTURED IN TWO PLACES. A HOIST SHAFT PEDASTAL BEARING CAP FAILED CAUSING THE BRAKE TO JAM.
