WHE N OPERATOR STATEDTO GO BACK TO WORK HE RELASED BRAKE, LET UP ON DECELERATOR NOT REALIZING HE WAIN NEUTRAL & ROLLED OVER RIDGE OF DIRT & HIGHWALL. NO DAMAGE TO DOZER.
THE EMP. WAS ON ONE SIDE OF THE PLANT REPAIRING A LEAKING WATER LINE. ON THE OTHER SIDE OF THE PLANT A WELDER WAS FABRICATING AND WELDING NEW LINES. THE CONT. GLARE FROM THE WELDERS ELECT RODE CAUSED MECHANI CS EYES TO BECOME FLASH BURNT.
FOREMAN WAS STANDING ABOUT 20 FEEET TO ONE SIDE OF DOZER A TREE CAME AROUND THE SIDE OF THE BLADE KNOCKING HIM DOWN FRACTURING THE FIBULA BONE IN LEFT LEG
