VICTIM WAS WORKING ON ROOF BOLTER, USING A WRENCH. HELPER ACCIDENTLY HIT A LEVER ON THE MACHINE WHICH CAUSED IT TO MOVE, SLINGING THE WRENCH OFF AND STRIKING VICTIM ON JAW.
