EMPLOYEE WAS IN THE PROCESS OF INSTALLING ROOF BOLTS WHEN THE WRENCH TAT HE WAS HOLDING THE ROOF BOLT WITH BROKE STRIKING EMPLOYEE IN THE HEAD CAUSING A LACERATION OF LEFT EYE WHICH REQUIRED EIGHT STITCHES.
