HE WAS USING CUTTING TORCH TO REMOVE GALVANIZED STEEL GUARDRAIL POSTS FROM BRIDGE AND INHALED THE FUMES CAUSING CHEST PAINS.

EMPLOYEE WAS CLEANING AROUND THE ENGINE AREA WITH A RAG. AS HE PASSED NEAR THE ALTERNATOR, WHICH HAD AN ACCEPTABLE BELT GUARD IN PLACE, THE CORNER OF THE RAG GOT SUCKED UP IN THE BELT AND JERK ED HIS ARM BACKWARD CAUSING PULLED MUSCLES IN THE UPPER ARM. THIS ACCIDENT IN NO WAY INVOLVED A VIOLATION OF RULES AND REGULATIONS OR JOB SKILLS, MINER PROFICIENCY, TRAINING AND ATTITUDE.

WAS OPERATING D2ZER UNDER NORMAL CONDITIONS WHEN OPERATOR COMPLAINED ABOUT HAVING LOWER BACK PAINS. THE CAB WAS MISSING SOME MOUNTING BOLTS CUASING SOME VIBRATION OF THE CAB.
