Employee broke a drill steel off in the top, leaving approximately 8 inches of the steel handing out. After drilling another hole for the roof bolt, he caught his left index finger between the roof plate and the broken drill steel while installing a roof bolt.

EE WAS SWINGING A BOLTER WRENCH WITH HIS RIGHT HAND STRIKING AGAINST A PLUGGED UP BOLTER STEEL, WHEN IT GLANCED OFF AND HE STRUCK HIMSELF WITH A WRENCH ABOVE HIS RIGHT EYE.

A ROOF FALL OCCURRED DURING MINING IN THE 5-LEFT WORKING SECTION WHILE CONNECTING THE XCUT BETWEEN NO. 7 AND NO. 8 ENTRIES. THE ROOF FALL RODE BACK THROUGH THE INSTALLED ROOF BOLTS, STRIKING T HE REMOTE CONTROLLED CONTINUOUS MINER OPERATOR AND MINER HELPE. THE MINER OPERATOR SUSTAINED SERIOUS PERSONAL INJURIES WHILE THE MINER HELPER SUSTAINED FATAL INJURIES.
