Fletcher operator attempted to pull in to XC-42, between 5 & 6 Entry, the Fletcher contacted the mobile dust pod already in that crosscut and also contacted an employee working on cleaning the Pod. The contacted employee was caught between the two units, resulting in bruising to the back and legs.

While tensioning truss cables, used in the Minova In-Roof truss system, using the truss gun one of the truss shoes broke free from the roof causing the truss gun, cable and dog bone keepers to fall and strike the employee on the right shoulder. Employee received chiropractic care.

Employee was standing on Shield 176 and was operating the pan push function, using the in-shield controls. EE left hand was resting on the gob wing. As the TG drive pushed forward, ee middle finger on the left hand was pinched by the gob plate as it moved closer to the shields during the push.

While mining in #3 the Cutter Head contacted a rib pin causing the washer to break free. The washer projected back 50' at the operator striking EE in the forehead. Resulting in a laceration above the left eye.

During roof bolting operations the wrench became stuck on the head of a roof bolt. While attempting to remove the wrench the miner's left index finger was caught between the drill mast and the wrench resulting in a laceration.

Employee claims he was roof bolting & installing monkey face plates in the 402 beltline. As he attempted to install a bolt it bent & he removed the bolt from the wrench & he re-attached the wrench to try to straighten the bolt. As he was doing this the plate hung up & then released & swung around striking him in the back of his right hand.

While performing PM service per work order; Transmission Oil Sampling. Service Mechanic was attempting to attach sampling bottle to port, cock R. Elbow and same Elbow was struck by a direct drive cooling fan. Resulting injury was a 1 inch laceration. Work was stopped and worker sent for Medical Attention.

While mining in 302 LW, a shield started to drop from the roof with out warning. Employee was walking toward the shield to reset it to the roof, the next shield (closest to TG) started to drop. Rocks and other materials started falling from the top of shield hitting employee's right leg, causing abrasion and intense pain in his knee.

Employee was lifting the roof bolt to place it in the wrench, the bolt was accidentally spun. the spinning bolt caught onto employee's gloved hand & twisted & pulled his finger around the bolt. Employee was diagnosis with dislocation to left ring finger and received stiches. Employee was released to restricted duty from Reliance Medical Group, LLC.

The next day while driving to work felt eye still sore. At the end of that day, employee went to Urgent Care facility after work. They referred her to a specialist, who gave her prescription med. On the third day, she told her supv.

A ROOF BOLTER OPERATOR ON THE H/G 101 SECTION WAS INSTALLING THE WRENCH ONTO A ROOF BOLT. HE INADVERTENTLY USED THE WRONG LEVER TO RAISE THE MAST. IN DOING SO THE OPERATOR ACTIVATED THE CLOSE JAW FUNCTION ON THE DRILL GUIDE SYSTEM. THE OPERATOR'S FINGER WAS IN A POSITION THAT ALLOWED THE JAWS TO CLOSE AROUND HIS FINGER CAUSING THE INJURY AND REQUIRED SOME STITCHES.
