Employee operating portable hydraulic jack pinched left pinky finger between the jack handle and the shop floor from not engaging the hydraulics and not maintaining control of employee's balance. The left pinky finger was broken.

Fletcher operator attempted to pull in to XC-42, between 5 & 6 Entry, the Fletcher contacted the mobile dust pod already in that crosscut and also contacted an employee working on cleaning the Pod. The contacted employee was caught between the two units, resulting in bruising to the back and legs.

While tensioning truss cables, used in the Minova In-Roof truss system, using the truss gun one of the truss shoes broke free from the roof causing the truss gun, cable and dog bone keepers to fall and strike the employee on the right shoulder. Employee received chiropractic care.

Employee was standing on Shield 176 and was operating the pan push function, using the in-shield controls. EE left hand was resting on the gob wing. As the TG drive pushed forward, ee middle finger on the left hand was pinched by the gob plate as it moved closer to the shields during the push.

While operating dozer, it hit a rough spot while changing, jolted the operator around causing pain in lower back.

While mining in #3 the Cutter Head contacted a rib pin causing the washer to break free. The washer projected back 50' at the operator striking EE in the forehead. Resulting in a laceration above the left eye.

While operating the shearer the operator was struck by a chunk of coal that bounced off the back of the panline. The lump struck against the side of the EE's left leg.

Employee was struck on the arm by a transport manifold after employee had raised a shield to the roof on the set up face. Resulted in a lacerated forearm.

Employee was cleaning crusher with an air chipper and caused her pain to her left elbow.

Employee fractured the tip of his left index finger while installing roof bolts in 403. Employee received 4 stitches and released to restricted duty.

During roof bolting operations the wrench became stuck on the head of a roof bolt. While attempting to remove the wrench the miner's left index finger was caught between the drill mast and the wrench resulting in a laceration.

Employee claims he was roof bolting & installing monkey face plates in the 402 beltline. As he attempted to install a bolt it bent & he removed the bolt from the wrench & he re-attached the wrench to try to straighten the bolt. As he was doing this the plate hung up & then released & swung around striking him in the back of his right hand.

While performing PM service per work order; Transmission Oil Sampling. Service Mechanic was attempting to attach sampling bottle to port, cock R. Elbow and same Elbow was struck by a direct drive cooling fan. Resulting injury was a 1 inch laceration. Work was stopped and worker sent for Medical Attention.

While mining in 302 LW, a shield started to drop from the roof with out warning. Employee was walking toward the shield to reset it to the roof, the next shield (closest to TG) started to drop. Rocks and other materials started falling from the top of shield hitting employee's right leg, causing abrasion and intense pain in his knee.

Employee got his foot caught between the relay bar and the large clevice that attaches to the headdrive. Employee's toe was pinched.

Employee was lifting the roof bolt to place it in the wrench, the bolt was accidentally spun. the spinning bolt caught onto employee's gloved hand & twisted & pulled his finger around the bolt. Employee was diagnosis with dislocation to left ring finger and received stiches. Employee was released to restricted duty from Reliance Medical Group, LLC.

While dusting beltline, dust hose surged and dust got into employee's right eye. Employee was diagnosis with corneal abrasion and was prescribed a medication with a full duty release.

Initial injury was reported on 8-25-07, while removing an air operated grease pump from fixture. Follow up visit on the 27th resulted in restricted duty.

Employee smashed finger between a jack hammer & cable trough while breaking rock on longwall face. He was evaluated at medical facility & diagnosed with left 5th finger laceration & required sutures.

Employee was power washing an equipment. She stated her eyes became irritated at the end of the shift, so removed contact lenses and washed eye out. The next day while driving to work felt eye still sore. At the end of that day, employee went to Urgent Care facility after work. They referred her to a specialist, who gave her prescription med. On the third day, she told her supv.

Employee pinched hand between bolt head and plate while installing a bolt. He was transported to medical facility and received suture to right hand at thumb and web.

Employee's left index finger was caught between oxygen bottle and railing in weld shop. Employee was seen at Reliance Medical and received 8 stitches.

Employee pinched his hand between drill steel extension and the drill steel. Employee received five sutures to his right hand and was released full duty with no restriction.

The winch on the coal car broke and struck EE's right hand causing laceration. Employee received stitches at Mesa Occupational & Sports Medicine Clinic.

EE (ROOF BOLTER OPERATOR) WAS STRUCK IN THE HEAD BY A TRUSS ANCHOR SHOE DURING INSTALLATION OF THE TRUSS. THE TRUSS LEG BROKE DURING INSTALLATION AND THE SHOE WAS THROWN OFF STRIKING THE OPERATOR ABOVE THE LEFT EYE, RESULTING IN A LACERATION APPROX 3" LONG

Employee placed his hand between drill box and a drill steel that was hung up in the roof. The steel fell unexpectedly, fracturing employee's middle finger.

When the cable reel was turned on, employee caught her left leg between the cable and rib, resulting in a contusion

EMPLOYEE WAS SUCKED IN BY THE VACUUM WHILE HANGING THE VENT TUBE CAUSING HIM TO HIT HIS CHEST AND CHIN AGAINST THE TUBE. RESULTED IN LACERATION INSIDE AND OUTSIDE OF HIS MOUTH.

HE STRAINED HIS LEFT ELBOW WELDING.

END DUMP OPERATOR STATED THE SHOVEL OPERATOR HIT HER TRUCK BED WHICH JOLTED HER AROUND IN THE CAB. IT WAS NOT DETERMINED IF IT WAS THE SHOVEL BUCKET OR A LARGE ROCK THAT HIT THE BED. THE INJUR ED WAS DIAGNOSED WITH CERVICAL AND THORASIS MUSCLE STRAIN.

WHEN EE FINISHED TIGHTENING THE COUPLER WITH THE BATTERY DRILL, THE DRILL CONTINUED TO ROTATE CAUSING IT TO FLIP AND TWIST EMPLOYEE'S WRIST. EE WAS TAKEN TO MESA CLINIC AND THE DR. DIAGNOSED H ER TO HAVE A FRACTURED WRIST.

EE HYPER-EXTENDED HIS RIGHT KNEE, WHEN HE STEPPED ONTO THE BOOM PLATFORM THAT WAS LOWERED WHILE ROOF BOLTNG

EMPLOYEE WAS INJURED WHILE WORKING WITH THE BULK ROCK-DUSTING SYSTEM. HE ATTEMPTED TO BLEED DOWN THE PRESSURE IN THE DUST TANK, AN UNSECURED VENT HOST SLID OUT OF THE STOPPING AND STRUCK HIM O N THE SIDE OF HIS FACE, CAUSED A LACERATION TO HIS FOREHEAD, CHIPPING A TOOTH AND CORNEAL ABRASION.

EE WAS AIR ARCHING OFF ANGLE IRON, ANGLE IRON BREAKING FREE AND HANGING. GRABBED ANGLE IRON TO BREAK FREE, DROPPED WHILE HOLDING ON TO FEDDER PAN. RESULTED IN LACERATION REQUIRING 6 STITCHES.

EMPLOYEE WAS IN THE PROCESS OF CHANGING DOZER TRACK, TRACK SEPARATED BEFORE HE WAS READY. THE HANDLE OF THE IMPACT WRENCH SCRAPED HIS SHIN.

WHILE ROOF BOLTING EMPLOYEE'S HAND CAUGHT BETWEEN A DRILL GUIDE CLAMP AND PINCHED HIS HAND.

EE WAS OPERATING A TRACK DOZER, PUSHING OVERBURDEN TO AN ELECTRIC SHOVEL, WHILE IN REVERSE MOTION, THE EE HIT A LARGE ROCK THAT JOLTED HIM AS HIS DOZER WENT OVER IT.

EMPLOYEE WAS PERFORMING THE PRE-PM TASK OF STEAM CLEANING WHEN HE ACCIDENTALLY BRUSHED THE STEAM CLEANING WAND ACROSS HIS LEFT FOOT RESULTING IN SECOND DEGREE BURN TO THE TOP OF LEFT FOOT.

WHILE BACKING DOZER AND LOOKING OVER SHOULDER, DOZER BACKED INTO AN UNEVEN AREA, CAUSING MACHINE TO JERK. OPERATOR TWISTED AND FELT PAIN IN UPPER BACK.

EE WAS INSTALLING ROOF BOLT. AFTER TIGHTENING, EE DROPPED THE DRILL HEAD AWAY FROM THE ROOF BOLT AND THE WRENCH FLEW OUT OF THE DRILL HEAD AND STRUCK EE IN THE MOUTH.

EE'S FINGER CAUGHT BETWEEN THE ROOF BOLTER DRILL MAST & DRILL STEEL THAT FELL OUT OF THE DRILL HOLE, CAUSING A FRACTURE TO HIS FINGER TIP.

EMPLOYEE WAS CUTTING PIECE OF IRON, IRON FELL AND STRUCK FOOT CAUSING FRACTURE. STARTING LOSING TIME.

POCKET OF METHANE IGNITED BY CUTTING TORCH CAUSING FLASH BURN TO HANDS, WRISTS, & FACE.

EE WAS WORKING ON LEFT HAND DRILL MAST. STEEL WAS STUCK IN THE ROOF. EXTENSION WAS IN DRILL HEAD, ABOUT 12" BETWEEN STEELS. EE REACHED UP TO PULL STEEL FROM ROOF, WHEN IT CAME LOOSE, IT PINCHED HIS LITTLE FINGER BETWEEN THE STEELS, CAUSING AN INJURY TO THE TIP OF FINGER.

A ROOF BOLTER OPERATOR ON THE H/G 101 SECTION WAS INSTALLING THE WRENCH ONTO A ROOF BOLT. HE INADVERTENTLY USED THE WRONG LEVER TO RAISE THE MAST. IN DOING SO THE OPERATOR ACTIVATED THE CLOSE JAW FUNCTION ON THE DRILL GUIDE SYSTEM. THE OPERATOR'S FINGER WAS IN A POSITION THAT ALLOWED THE JAWS TO CLOSE AROUND HIS FINGER CAUSING THE INJURY AND REQUIRED SOME STITCHES.

EE WAS DRILLING DRILL BIT CAUGHT A PIECE OF RBARCAUSING DRILL TO JERK, SPRAINING WRIST

EE WAS MOVING CROSSOVER TO ALIGN WITH TRACK OF BOLTER AND FELT A PULL IN THE LOWER BACK.

EE WAS PUTTING DRILL HAMMER INTO DRILL RIG, RIG PINCHED HAND BETWEEN RIG & RIG STEP. HAND PINCHED OR MASHED CAUSING FRACTURED FINGER ON RIGHT HAND.
